外科手技時の手袋破損の発生率:二重手袋/インジケータシステム手袋のベネフィット★★

背景

手術中に手袋を使用する目的は、手術部位感染症(SSI)を予防し、スタッフと患者の両方を交差汚染から保護することにある。

目的

手術用手袋破損率を評価し、それらの破損により感染リスクが上昇するかどうか、また上昇するのであればそれを克服する方法(二重手袋法/インジケータシステム手袋法の使用)を検討した。

方法

目的において特定された主要研究項目を検討するために、スコーピングレビューを実施した。検索方法としては、PubMed データベースを用いて、1970 年 1 月 1 日から 2024 年 7 月 31 日までを対象として「surgical(手術) AND gloves(手袋) AND perforations(穿孔)」により検索を行った。2 名の異なるレビュアーが、PRISMA-Scr チェックリスト法を用いて独立して論文をスクリーニングした。

結果

最初の検索により論文 343 報が特定され、続くスクリーニングにより 129 報がフルテキストレビューの対象となった。その結果、手術用手袋の穿孔は、様々な異なる外科手技において様々なレベルで高頻度に報告されているが、重要なことに、高い割合の手袋破損が気付かれないままであることが示された。これらの穿孔の発生率は様々であるが、データの解析から、整形外科、口腔顎顔面外科および産科/婦人科が最も高い発生率に関連していたことがわかり、これはおそらく、手袋を損傷しやすい機械的手技の使用が原因であると考えられる。細菌伝播および SSI の発生率上昇は、手術用手袋穿孔の頻度の増加と関連している。インジケータシステム手袋の使用は、手術用手袋破損の特定という点において、大きなベネフィットを示した。

結論

高リスクの外科手技において、インジケータシステムを用いると、穿孔/破損の検出によって外科医と患者の両方に追加の保護が提供され、細菌伝播と交差感染が防止される。

監訳者コメント

手術中にインジケータシステムの採用された『手術用手袋』を用いると、穿孔/破損が起こっても、早期に発見することが可能であり検出細菌伝播と交差感染のリスクを少なくすることができ推奨される。
