集中治療室の環境では「high touch（高頻度接触）」表面が常に「high risk（高リスク）」表面ではない★★

目的
この研究は、集中治療室（ICU）の環境での病院関連感染の媒介物経路による伝播において高リスクである品目を特定することと、細菌によるそれらの汚染のレベルを評価することを目的とした。
方法
表面への手の接触についての 15 日間の構造化された観察を、中国の杭州にある 3 次病院の 13 床の ICU で実施した。ネットワークの中心性に基づいて高リスク品目を特定するために、接触のデータから有向加重ネットワークを構築した。好気性菌を、高頻度接触表面、低頻度接触表面、高リスク表面、低リスク表面を含む環境表面から、4 日にわたって、9：00、13：00、17：00、20：00に採取した。
結果
高頻度接触品目と高リスク品目の間にいくつか重複はあったが、高頻度接触品目が、一貫して、高リスク品目として特定されたわけではなかった。細菌による高リスク品目表面の汚染のレベルは、低リスク品目表面よりも有意に高いことが分かったが、高頻度接触品目と低頻度接触品目の間で、細菌による汚染のレベルに有意差は観察されなかった。
結論
ネットワーク解析によって高リスク品目を特定することによって、ICU の環境における表面のクリーニングおよび消毒の戦略がより正確なものになるように導くのに役立つ理解がもたらされる。高頻度接触品目は、重要と考えられることが多いが、常に、より汚染されているわけではない。
監訳者コメント：
接触頻度が高い環境表面が必ずしも細菌汚染の高リスクな環境表面とは限らず、病床間を移動する物品が汚染源となりうる。All Touchable Surfaces に対して、環境衛生が重要であることが示唆された。
