バイオエアロゾル処理および COVID-19 低減策による医療関連感染症への効果★★

背景
呼吸ゾーンの空気の浄化および新型コロナウイルス感染症(COVID-19)の低減策が医療関連感染症に及ぼす実臨床での効果は、十分に裏付けられていない。疾患の空気伝播に対処する工学的解決策は管理された試験室または個室では成果をもたらす可能性があるが、病院全体での適用に関しては報告されておらず、COVID-19 低減策が医療関連感染率に及ぼす効果は不明である。
目的
病院全体でのバイオエアロゾル処理と COVID-19 低減策が臨床転帰に及ぼす効果を明らかにすること。
方法
空気消毒技術および COVID-19 低減策を標準的・多面的感染制御に段階的に追加することによって、粒子数、ウイルスおよび細菌のバイオバーデン、医療関連感染率に対して及ぼす効果を、124 床の病院 1 施設(30か月にわたり > 100,000患者日)において検討した。
結果と結論
空気消毒技術および COVID-19 低減策の追加によって空気中の超微細粒子が減少し、病院のバイオバーデンが変化し、医療関連感染症が 1,000 患者日あたりそれぞれ 11.9 から 6.6 に、6.6 から 1.0 に減少した(P < 0.0001, R2 = 0.86)。単独で医療関連感染症を削減する技術、ツールまたは方法はないだろうが、広範な施設全体の工学的解決策の追加によって、わずかな費用で、また患者、訪問者またはスタッフの通行や作業の流れのパターンを変えることなく、感染症を 45%減少させた。包括的、制限的、労働および材料集約的な COVID-19 低減策の追加によっても同様の効果が裏付けられた。著者らの知る限り、本研究は従来型の感染制御、工学的解決策および COVID-19 低減策を初めて直接比較したものである。
監訳者コメント:
COVID-19 のパンデミックは我々のこれまでの感染予防策を変更せざるをえない状況をもたらした。特にエアロゾルによる感染は、フィジカル・ディスタンスやユニバーサル・マスキングに加えて、「換気」の重要性を再認識させた。本論文では病院の空調の「空気消毒技術」により院内の感染率を低減させたとの結論であり、今後このような論文が増え、病院そのものの建築基準も変わる可能性がある。
