背景

カルバペネム耐性アシネトバクター・バウマニ(Acinetobacter baumannii)(CRAB)感染症は症例致死率が高い。これらの感染症の発症率は上昇しており、治療選択肢は限られているが、CRAB の交差伝播を予防するための効果的な介入についてはほとんど試験されていない。

目的

CRAB 患者のコホーティングと強化された環境クリーニングの組み合わせが CRAB 血流感染症(BSI)に及ぼす影響を評価する。

方法

2022 年 1 月から2024 年 6 月に 3 次病院で実施された準実験的研究。内科に入院した CRAB(コロニー形成または感染のいずれか)を有する患者全員をコホーティングした。同時に、CRAB 患者が滞在しているすべての病室で、1 日 2 回のルーチンクリーニングと二重のターミナルクリーニングを実施した。毎月の CRAB BSI 発症率を算出し、CRAB 患者の病院への流入について制御した頑健な標準誤差を用いたポアソン回帰不連続解析により発症率比(IRR)を推定した。

結果

2022 年 1 月から 2024 年 6 月に CRAB を有する入院患者 610 例が特定され、そのうち 350 例(57%)は病院内で菌を獲得し、138 例(39%)が BSI を発症した。全 30 日死亡率の平均は61%で、研究期間を通してほぼ変化しなかった。累積 BSI 発症率は、介入前の 10,000 入院日あたり 1.43 件から介入後の0.65 件まで 55%低下した。BSI 発症率の傾きは 1 か月あたり9%減少した(補正 IRR 0.909、95%信頼区間[CI]0.834 ~ 0.990、P = 0.029)。

結論

内科での CRAB 患者のコホーティングと病院全体での強化されたクリーニングの組み合わせにより、施設全体での CRAB BSI 発症率が有意に低下した。

監訳者コメント

日本では病棟のハード面での制約(個室率の低さ)や人的資源の限界があるため、コホーティングの実現には工夫が必要である。一方で本研究の清拭プロトコル(1 日 2 回の日常清掃+2重ターミナル清掃)は、日本の医療現場でも実行可能で、CRAB血流感染という疾病負担を考慮すれば、医療経済的にも合理的な投資と考えられる。
