The impact of discontinuing single-room isolation of patients with vancomycin-resistant enterococci: a quasi-experimental single-centre study in South Korea

目的

ガウンや手袋の使用のような接触予防策を続けながら、個室隔離を中止することの影響についてのデータは限られている。2021 年 4 月、我々の病院は、使用できる個室がなかったために、バンコマイシン耐性腸球菌（VRE）を有する患者の個室隔離を中止した。この研究では、この方針の影響を、VRE による院内血流感染（HA-VRE BSI）の発生率を調べることによって評価した。

方法

この後向き準実験的研究は、韓国のソウルの 3 次医療病院で実施された。時系列分析を使い、方針の変更前（第一相）と変更後（第二相）における病院レベルおよび血液内科での HA-VRE BSI の発生率を評価した。

結果

病院レベルでは、HA-VRE BSI の発生率のレベル（VRE BSI / 1,000患者・日 / 月）や傾向に、第一相と第二相の間で有意な変化はなかった（それぞれ、係数 −0.015、95％信頼区間［CI］ −0.053 ～ 0.023、P ＝ 0.45、および 0.000、95％CI −0.002 ～ 0.002、P ＝ 0.84）。同様に、血液内科でのHA-VRE BSIの発生率のレベルと傾向（それぞれ、−0.285、95％CI、−0.618 ～ 0.048、P ＝ 0.09、および−0.018、95％CI −0.036 ～ 0.000、P ＝ 0.054）も、2 つの相で有意に変化しなかった。

結論

VRE が定着していた、または VRE に感染していた患者の個室隔離を中止することは、病院レベルや、血液内科の高リスク患者において、HA-VRE BSI の発生率の増加と関連していなかった。手指衛生や環境清掃の強化のような対策を含む多剤耐性病原体に対する水平の介入の方が、VRE の伝播の予防には有効かもしれない。

監訳者コメント：

韓国からの報告。血液内科病棟で VRE 保菌者の個室隔離を中止することで VRE に関連する血流感染は増えなかった。手指衛生や環境清掃の強化の方が、個室隔離より有効な可能性がある。日本では VRE の検出が少ないが、隔離できる個室が限られている。VRE 保菌者の隔離の影響とその必要性に関してさらなる検討が必要である。2024 年 4 月号にもデンマークからの同様の報告（Journal of Hospital Infection (2024) 146, 82-92）がなされている。こちらも合わせて参考にされたい。
