新生児集中治療室におけるメチシリン耐性黄色ブドウ球菌(Staphylococcus aureus)保菌についての COVID-19 パンデミック前後の感染管理対策強化導入後の比較:後向視的研究★

背景
新生児集中治療室(NICU)に入室している乳児は、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)を保持していることが多い。新型コロナウイルス感染症(COVID-19)パンデミック中、NICU の感染対策が強化され、主に、訪問制限や、来訪者に対する厳格な感染制御策であった。本研究では、COVID-19 パンデミック中に NICU で感染対策を強化したことが、乳児の MRSA 定着頻度に与えた影響を調査したものである。
方法
本後向視的研究では、2017 年 から 2022 年の間に当院のNICUに入室した乳児 1,066 例を対象とした。COVID-19 の感染拡大を受けて、2020 年 3 月以降、NICU における感染対策のバンドルが強化された。我々は、パンデミック前(2017 年 から 2019 年)とパンデミック後(2020 年 から 2022年)の期間におけるMRSA保菌率を比較した。
結果
1,066 例の乳児のうち、COVID-19 パンデミック前の期間に NICU に入院したのが578 例、パンデミック後の期間に入院したのが 488 例であった。NICUにおけるMRSA保菌乳児の割合は、パンデミック前後で有意差は認められなかった(7.8%対 10.8%)。多変量ロジスティック回帰分析の結果、感染対策が強化されたパンデミック後であってもMRSA 保持率に関連性は認められなかった。
結論
感染対策が強化されたにもかかわらず、COVID-19 パンデミック後の期間において、NICU におけるMRSA 保菌乳児の割合は減少しなかった。訪問者に対する感染対策を含む、より厳格な感染対策バンドルへの移行は、NICU の環境における MRSA 保菌率の減少に寄与する主要な要因ではない可能性がある。
監訳者コメント :
日本の大学病院からの報告である。本研究ではすでに標準的感染対策が確立された場合「マクロレベル」の面会制限より「ミクロレベル」のデバイス管理が重要であり、カンガルーケアを含む母子接触は不要に制限すべきでないことを示唆したものであり参考になり得る。
