新生児集中治療室(NICU)における院内感染管理分析:ロジスティック回帰と故障モード影響解析を組み合わせた後向きコホート研究★

背景
新生児集中治療室(NICU)における低出生体重児は、未熟な免疫と侵襲的な手技のために高い感染リスクに直面している。しかし、従来の事後対応型管理は時に失敗することがある。故障モード影響解析(FMEA)(定性的)およびロジスティック回帰(定量的)を統合して用いた研究はほとんどない。本研究では、両法を併用してリスクの優先度を明らかにし、管理策の検証を行い、その指針を提示した。
目的
本研究の目的は、NICU 内の症例の院内感染管理における FMEA とロジスティック回帰の適用について検討することである。
方法
NICU の新生児について、感染新生児 100 例を感染グループ、非感染新生児 200 例を対照グループに分類した。、単変量解析およびロジスティック回帰分析により、NICU における感染の重要なリスク因子を同定した。FMEA を、同定したリスク因子に対して適用し、介入の効果を評価した。
結果
NICU の院内感染における独立リスク因子は、出生体重 1,500 g 未満、在胎期間 37 週未満、入院期間 14 日超、人工呼吸器の使用、中心静脈カテーテル留置などの侵襲的手技、ならびに気管挿管などであった(P < 0.05)。FMEA の適用後、NICU における 2022 年の感染率(3.55%)は 2021 年の感染率(5.02%)より有意に低かった(P < 0.05)。その他のアウトカムは、平均入院期間の 14.2 日から 10.8 日への短縮(P < 0.001)、死亡率における変化なし(0.58%対 0.24%、P = 0.174)、ならびに多剤耐性病原体(MDRO)による感染率の 1.9%から 0.9%への低下(P = 0.014)および抗菌薬療法の実施期間の 7.5 日から 6.2 日への短縮(P < 0.001)などであった。
結論
回帰分析による統計結果に基づくと、FMEA を用いた管理は NICU における院内感染リスクの制御のために効率的かつ実施可能なツールであり、これによって感染管理担当者は、標的を絞った介入の選択、ならびにおそらく科学的な意思決定が可能になる。
監訳者コメント :
この論文は、NICU の感染対策を“勘や経験”から“構造的・データ駆動”へと転換した研究であり、シンプルながら実務的な応用価値が高い。NICU は感染ハイリスク環境であり、従来は問題発生後に対応する後追い型の管理が中心であったが、予防の観点からは不十分であった。
本研究では、ロジスティック回帰(定量)により「どの因子が真のリスクであるか」を数値的に特定し、FMEA(定性)により「どのプロセスで失敗が生じるか」を分析している。これらを組み合わせることで、感染対策に優先順位を付けた点が重要である。
