背景

結腸の手術および腹式子宮摘出術での手術部位感染症(SSI)と、手術中の手術室内の空気圧の変わりやすさの間の関係は十分に研究されていない。

方法

我々の後向きコホート研究では、全米医療安全ネットワーク(NHSN)の結腸のコードまたは腹式子宮摘出術のコードが割り当てられた手術を受けた患者を取り上げた。主要曝露は、圧が範囲から外れていた手術時間の割合とした。SSIの特定には、NHSNの基準を使った。曝露と SSI の間の関連を分析するために、多変量二項ロジスティック回帰モデルを使用した。より均質な患者(例えば、結腸の患者のみや、圧が 1 Pa 未満であったことが少なくとも 1 回あった患者のみ)から成る感度モデルも構築した。

結果

1,663 例の患者のうち、933 例(56%)が結腸の手術を受け、730 例(44%)が子宮摘出術を受けた。多変量について調整後、手術室の圧の逸脱と SSI の間に関連は検出されなかった(圧が範囲から外れていた手術時間の 1%の増加によるオッズ比[95%信頼区間]、1.05[0.46 ~ 2.24])。この関連のなさは、前述の感度モデルでも同様であった。

結論

手術室の空気圧の変わりやすさと、結腸の手術や子宮摘出術での SSI の発生との間に有意な関連は観察されなかった。

監訳者コメント

手術中の室圧が推奨値を維持できなかった時間と、SSI の発生リスクとの間に統計学的な有意差を認めなかった。陽圧の乱れが直ちに感染リスクを増大させるわけではないことを示唆している。
