背景

難民の健康や福祉は、感染症の出現と、抗菌薬の誤用を促進する厳しい生活状態によって危機的な状況となる。この多施設共同横断的地域密着型研究では、東地中海の国であるレバノンにある仮設キャンプで暮らすシリア人難民における尿中の保菌率、およびそれに関連する分離株の抗菌薬耐性パターンを調べた。

方法

この感染しやすい集団において、細菌尿と関連するリスク因子を特定するために、多変量ロジスティック回帰モデルを使った。尿路感染の症状を示しているかどうかに関係なく、79 人のシリア人難民から中間尿検体を無菌的に採取した。検体は、発色寒天培地 UriSelectTM 4 に播種し、分離株は、MALDI-TOF-MS を使って同定した。抗菌薬感受性試験は、Kirby-Bauer ディスク拡散法を使って実施した。

結果

細菌尿の罹患率は、30.8%(95%信頼区間、21.6 ~ 41.7)で、病原体は、大腸菌(Escherichia coli)(80%)、肺炎桿菌(Klebsiella pneumoniae)(12%)、エンテロバクター・クロアカエ(Enterobacter cloacae)複合体(8%)が主であった。腸内細菌目細菌分離株は、ペニシリン系(64%)、セファロスポリン系(20 ~ 48%)、キノロン系(28%)に対して高度の耐性を示し、56%が多剤耐性を示した。女性であることと、ごみの堆積がリスク因子であった一方で、紅茶の摂取、鶏肉の摂取量を減らすこと、経済的支援が、細菌尿を予防した。

結論

この報告は、レバノンのシリア人難民における診断されていない細菌尿についてこれまでにあった事例証拠を裏付けている。データは、難民と受け入れ側の地域社会の両方において、感染症と抗菌薬耐性が広がるのを抑えるための監視および啓発プログラムが緊急に必要とされていることをはっきりと示している。

監訳者コメント

難民における細菌尿の頻度、分離された腸内細菌目細菌の多剤耐性化が明らかになった。
