背景

再処理プロトコルが遵守されているにもかかわらず、十二指腸内視鏡には、依然としてしばしば汚染がある。これは、再処理の効率についての知識に著しいギャップがあることをはっきりと示している。

目的

十二指腸内視鏡再処理の手順において、汚染率に影響を与えるリスク因子を特定すること。

方法

2022 年 2 月から 2023 年 12 月の間に Pentax ED34-i10T2 十二指腸内視鏡から採取した培養を組み入れた。汚染は、腸や口腔由来の微生物の存在によって明らかにした。十二指腸内視鏡の使用、再処理のリードタイム、再処理に関わる職員についてのデータを、電子カルテから取り出した。リスク因子は、再処理ガイドラインおよび文献から得た。これらには、手動での洗浄の開始の遅れが 30 分を超えること、手動での洗浄の時間が 5 分以下、乾燥時間が 90 分未満、職員の再処理に関わる頻度、7 日間を超える保存、等があった。十二指腸内視鏡の汚染に対するこれらの因子の影響を、ロジスティック混合効果モデルで評価した。

結果

307 個の十二指腸内視鏡培養のうち 58 個(18.9%)は、腸や口腔由来の微生物によって汚染されていた。研究期間全体で、十二指腸内視鏡は、再処理を 1,296 サイクル受けた。手動での洗浄の時間が 5 分以下だと、汚染のオッズが有意に増加した(補正オッズ比[aOR]1.61、95%信頼区間[CI]1.10 ~ 2.34、P = 0.01)。十二指腸内視鏡の使用の増加は、汚染のオッズの減少と関連していた(aOR 0.80、95%CI 0.64 ~ 0.995、P = 0.045)。調べたその他のリスクは、汚染率との明らかな関連を示さなかった。

結論

手動での洗浄の時間が 5 分以下だと、腸や口腔由来の微生物による汚染のオッズが増加した。再処理の開始が遅れることと不完全な乾燥は、従来リスク因子と考えられているが、この研究では、汚染のリスクの増加と関連がなかった。今後の研究では、再処理の時間の監視を強化することで十二指腸内視鏡の汚染を軽減することができるかどうかを調査すべきである。

監訳者コメント

十二指腸内視鏡の手動での洗浄時間が 5 分以下だと微生物による汚染オッズが上昇するという結果がでている。洗浄時間は作業の勤勉さと完全性の指標になる可能性がある一方で、洗浄スタッフの習熟度により洗浄時間が短くなることもある。単施設での報告であり、結果の一般化には限界があるだろう。
