背景
面接やアンケート調査は、面接、アンケート、専門家パネルを使い、勧告やケアバンドルの遵守に焦点を当てることによって、手術部位感染症（SSI）の予防における障壁や課題を特定してきた。この研究は、焦点を広げることを可能にする臨床業務の観察と面接を使うことによって、より網羅的な調査を提案する。
目的
心臓手術を例として使い SSI の予防に影響を与える要因を網羅的に特定すること。
方法
調査は、イングランドの 4 つの心臓センターにおける臨床業務の 130 時間の観察と、それに続く 16 名の外科医・麻酔科医・手術室スタッフ・看護師との個別の半構造化面接から成っていた。データの主題分析を実施した。
結果
要因は複雑で、介入、個人、チーム、組織のレベル、更には、それより広い社会のレベルにも存在していた。要因には、介入の属性、エビデンス・個人的信念・知覚リスクの間の関係、力とヒエラルキー、リーダーシップと文化、医療資源、インフラストラクチャー、供給、組織と計画、患者エンゲージメントと力、病院管理、従業員不足、COVID-19 パンデミック、ブレグジット（イギリスの EU 離脱）、ウクライナでの戦争、等があった。
結論
これは、SSI の予防に影響を与える要因についての網羅的な概観を提供する初めての調査の 1 つである。要因は複雑で、SSI を減少させようと試みる際には、十分に理解する必要がある。強力なエビデンスベースは、介入を確実に実行させるのに不十分であった。
監訳者コメント ：
イングランドでの手術部位感染症（SSI）の予防に影響を与える要因についての観察と面接を使った定性的調査である。SSI 予防に影響を与える要因は多岐に渡るが、スタッフの面接での発言をみると国による違いよりむしろ日本との共通点が多いことに気付く。思わず何度も「あるある」と頷いてしまった。
