背景
マイコバクテリウム・キマイラ(Mycobacterium chimaera)は、水を用いるヒータークーラーユニット内に定着し、そこから手術時に患者へと拡散することがある。Vermamoeba vermiformis は水中に生息する自由生活性のアメーバで、ヒータークーラーユニット内に常に存在している。
目的
これらのアメーバが、M. chimaera の持続的な存在に関与するかどうかを検討すること。
方法
In-vitro 消毒モデル。
結果
- vermiformis の存在下において、塩素曝露後に M. chimaera の生存延長が認められた。共焦点顕微鏡により、V. vermiformis の細胞内に M. chimaera の存在が確認された。
結論
以上のように、V. vermiformis はヒータークーラーユニット内における M. chimaera の持続的存在に寄与する可能性がある。清掃および消毒のプロトコールでは、この現象を考慮に入れるべきである。
監訳者コメント:
Mycobacterium chimaera は非結核性抗酸菌(NTM)のひとつで、水性環境に存在し、バイオフィルムを形成する。Vermamoeba vermiformis は、環境中に存在する水系の自由生活アメーバで、Legionella pneumophila やいくつかの NTM の宿主として機能できることが知られている。本研究では M. chimaera が実際に V. vermiformis 内に存在することが明らかにしている。また、vermiformis シストは塩素消毒に耐性があり、手術室のヒータークーラーユニット内を手順通りに清掃、消毒しても術後感染をきたしうることは知っておいたほうがよい。
