Evaluating the efficacy of action-based instructions on the suitability of medical protective masks in healthcare settings

背景

医療用防護マスクの空気の漏れは臨床現場において依然として無視できない問題であり、漏れによる不十分な防護は、不必要な職業性呼吸器感染につながる可能性がある。

目的

本研究は、行動介入の実施において看護スタッフの医療用防護マスク着用遵守に影響する因子を明らかにすることを目的とした。行動介入の有効性を評価して、看護スタッフに対する正しいマスク使用の指針を提供するために、定量的フィットテストを実施した。

方法

中国福利会（China Welfare Institute）の International Peace Maternity and Child Health Hospital の看護師 40 名からなるサンプルを選択して、医療用防護マスクの 3 つの着用法について観察し、ついで行動介入を実施した。定量的フィットテストを介入の前後に実施し、医療用防護マスクの合格率を記録した。

結果

簡単な手順法とフィットテストの組み合わせは、適正なマスク着用率の促進と保護の改善を可能にする。着用者の頭部・顔面の大きさに対するマスクモデルの適合性と快適さは、マスク使用の有効性と遵守率に影響するであろう。

結論

簡単な手順法による介入およびフィットテストの使用を動的場面シミュレーションによる評価と組み合わせることで、マスク着用の合格率を改善する科学的かつ実施可能な解決策が提供され、呼吸器保護の領域において「静的遵守」から「動的適応」へというパラダイムシフトが促進された。

中国での医療用防護マスクの適合性に関する行動に基づく指導の有効性の研究。医療従事者の 90％が医療用防護マスク使用に関するトレーニングを受けていたにもかかわらず、専門家による調整なしにマスクを装着した際に定量フィットテストに合格したのはわずか 15％で、既存の文献と同様に、医療現場で医療従事者が着用する医療用防護マスクの保護効果は予想レベルを下回っていることを示している。本研究ではマスクの適合性や快適さが与える影響についても検討しており、骨格が類似している日本人においても参考になるだろう。
