背景

レジオネラ・ニューモフィラ(Legionella pneumophila)は、レジオネラ症を引き起こしうる水系細菌である。Legiolert®(IDEXX、米国)は、水系の L. pneumophila(血清群[SG]1-15)の検出および計数を目的とした労働集約性の低い液体培養アッセイである。

目的

Legiolert と ISO 11731:2017 平板培養法(選択寒天培地における膜ろ過法および培養)の一致度を、病院の水サンプル(N = 100)を用いて分析すること。

方法

培養を、7 日間にわたりそれぞれ 39°Cおよび 36°Cで行った後、Legiolert について最確数法による計数と、平板培養により疑われる Legionella コロニーの継代培養および血清群判定を行った。

結果

L. pneumophila(SG1-15)が、Legiolert または平板培養を用いた場合、サンプル 100 個中 25 個で分離された。Legiolert により、さらにサンプル 14 個が陽性であり、同じサンプルの平板培養による分析では陰性(14 個中 12 個)、または Legionella rubrilucens のみ(14 個中 2 個、マトリックス支援レーザー脱離イオン化飛行時間型質量分析法[MALDI-TOF MS]により確認)が検出された。L. pneumophila は、これらの陽性サンプルについてQuanti-Tray/Legiolertのパウチのウェルから、buffered charcoal yeast-extract(BCYE)寒天培地における穿刺サンプルの継代培養後には検出されなかった。両法により一致して、サンプル 100 個中 61 個で L. pneumophila は検出されなかった。

結論

Legiolert と平板培養法はいずれも、水サンプルから L. pneumophila を検出するための満足できる方法であり、いずれも L. pneumophila 分離株を、今回のサンプル集団の 25%において検出できた。Legiolert は、より速く結果が得られ、必要なリソースが少なく、労働集約性が低い。しかし、他の微生物との交差反応性のリスクが低い可能性がある。両法いずれも、レジオネラ症発症の予防において L. pneumophila のモニタリングが不可欠となる医療環境における水系の分析に適している。
