背景

The Radboudumcは、患者から提供される手術創治癒に関する情報を収集するためのスマートフォンアプリ(WondGezond)を開発した。

目的

手術部位感染症(SSI)の早期検出における同アプリの性能を評価するために、使いやすさおよびアウトカムを評価すること。

方法

2020 年 8 月から 2023 年 2 月にかけて、脊髄変性疾患または手根管症候群に対して手術による治療を受けた患者を手術 1 日後に登録し、Quick Response(QR)コードにより本アプリをダウンロードするよう依頼した。参加者は写真をアップロードし、自身の手術創の状態について 14 日にわたり毎日 4 つの質問に回答した。その後、参加者は SSI の疑いに対して治療を受けたか否かを回答した(参加者報告によるアウトカム)。神経外科医 2 名が、写真、およびSSI の疑いに関する質問票の回答について独立して評価した(医師評価によるアウトカム)。両アウトカム間の関係を、感度、特異度、ならびに陽性的中率(PPV)および陰性的中率(NPV)を算出して明らかにした。

結果

手術 2,009 件の実施後、QR コード1,695個が配布され、412個(21%)が読み取られた。全参加者中 232 例(56%)が 14 日間にわたる質問に回答し、このうち 22 例(10%)が SSI 治療を受けたと報告した。医師の評価により、15 件(7%)の SSI が同定された。一致が得られたのは 88% の症例であった。不一致症例 27 件中、偽陽性が 17 件、偽陰性が 10 件あり、結果として感度(33%)および PPV(23%)は低かったが、NPV(95%)は高かった。

結論

本スマートフォンアプリにより、医師に対して、リスクがある患者の経過観察のための手術創治癒および SSI に関する情報が提供された一方、手術創治癒に関する問題がない患者にとっては、(過剰な)抗菌薬療法および不必要な受診が減った可能性もある。しかし、参加者数が少なかったことと偽陽性結果があったことから、本アプリは現在の形態ではサーベイランスの目的には適していない。本アプリのさらなる検証が必要である。
