ナノシルバー/DCOIT 含有表面コーティングは救急処置室の表面において効果的かつ恒常的に微生物量を減らす★

背景
病院における患者近傍表面への微生物の定着は、医療関連感染症の感染源として重要な役割を果たす。ルーチンの消毒法では、病原体の短期の除去が得られるだけである。
目的
新規開発されたナノシルバー含有抗微生物コーティングについて、ルーチンの定期消毒作業におけるギャップを埋める目的で病院表面の微生物量の長期にわたる低減に果たす役割を検討すること。
方法
本前向き二重盲検試験では、レベル 1 の病院の救急部でルーチンに使用されている処置室 1 室の高頻度接触表面を、ナノシルバー粒子含有表面コーティング(ナノシルバー/DCOIT 含有表面コーティング表面:NCS)と、別の有機殺生物剤(4,5-ジクロロ-2-オクチル-4-イソチアゾリン-3-オン:DCOIT)を用いて処置し、別の 1 室の表面を、ナノシルバーおよびDCOIT の両成分を含有しないコーティングで処置して対照とした。表面の細菌汚染は、接触プレートおよび液体ベースのスワブを用いて、計 90 日の試験期間にわたり毎日調べた。培養後、総微生物数および微生物種について評価した。
結果
微生物サンプル計 2,880 個において、全細菌量における有意な減少が、NCS 処置室(中央値 0.31 CFU/cm2、四分位範囲 0.00 ~ 1.13)で、対照処置表面(中央値 0.69 CFU/cm2、四分位範囲 0.06 ~ 2.00)と比較して認められた(P < 0.001)。ナノシルバー/DCOIT 含有表面コーティングにより、重要な細菌量(5 CFU/cm2 超と定義)の相対リスクが 60%減少した(オッズ比 0.38、P < 0.001)。NCS 表面と対照表面との間で、微生物種の分布に有意差は認められなかった。
結論
ナノシルバー/DCOIT 含有表面コーティングは、臨床環境の高頻度接触表面における細菌量を持続的に減少させる効果を示した。
監訳者コメント:
ステンレス鋼は、その外観、耐久性、掃除のしやすさから、病院環境で一般的に使用されているが、抗菌特性がない。一方で銀や銅など金属コーティングには抗菌特性があり、医療現場にも応用されている。今回は環境表面での銀コーティングの効果を救急処置室で調べた研究である。人が行う環境の除染の効果は短期的なのでこうした技術を組み合わせることは有用であろう。
