Risk factors for non-isolation of patients admitted for pulmonary tuberculosis in a high-incidence department: a single-centre retrospective study

背景

肺結核は、空気感染する疾患であり、入院時に疑わしい例を特定し、個室に入院させ、空気感染に対する追加の予防措置を実施する必要がある。

目的

発生率の高い病院において、隔離されなかった肺結核の頻度を特定し、空気感染に対する追加の予防措置の実施の遅れに関連する因子を特定すること。

方法

この後向き観察研究には、結核菌(Mycobacterium tuberculosis)が陽性の検体が少なくとも 1 つあった患者を組み入れた。患者の人口統計学的データ、臨床データ、入院の仕方に関係するデータを収集した。単変量および多変量統計解析を実施した。

結果

研究期間中、256 例の患者が組み入れられた。そのうち、134 例(52.3%)に肺結核があった(75%が男性、年齢の中央値は 39 歳、70%が外国生まれ)。これらの患者のうち、46 例(34%)が、入院後 24 時間を過ぎてから隔離された。空気感染に対する追加の予防措置の実施までの時間の平均は 4.3 日で、7 例の患者(5.2%)は、入院期間中、隔離をされなかった。多変量解析では、3 つの因子が隔離と関連していたことが示された。一般開業医の診察を受けていたことは、隔離の可能性が大きくなることと関連していたが、救急部門経由での入院は、関連していなかった。いわゆる主要な臨床徴候の存在と、示唆的な胸部X線写真も、隔離の可能性が大きくなることと関連していた。最後に、ヨーロッパ人の患者は、外国生まれの患者よりも、隔離される頻度が少なかった。

結論

この研究では、肺結核で入院した患者の 34%が、入院時には隔離されていなかった。隔離されない可能性が、救急部門経由で入院した症例では 3 倍高かった。また、ヨーロッパ人の患者は、外国生まれの患者よりも、隔離される頻度が少なかった。主要な徴候の存在と一般開業医の診察を受けていたことが、隔離の可能性が大きくなることと関連していた。

監訳者コメント

フランスからの報告。肺結核は空気感染し、早期に診断し、空気予防策を追加することが重要であるが、しばしば診断の遅れが院内感染の原因となりうる。日本とフランスでは結核の疫学が異なるが、隔離が遅れる原因には臨床兆候と画像所見が重要であることなど様々な示唆があり、参考になる。
