COVID-19 パンデミック後の医療関連感染の長期的変化:全国的な分割時系列解析★

背景
医療関連感染(HAI)は、世界全体で罹患および死亡の主な原因となっている。COVID-19 パンデミック中、医療施設は診療と収容能力の両面で大きく混乱した。このような混乱がパンデミック後の HAI に与えた長期的な影響は明らかにされていない。
目的
本研究の目的は、COVID-19 パンデミック後の HAI による入院における全国的な時空的傾向を調査し、関連する患者および医療要因を特定することであった。
方法
全国の入院データを用いて、毎月の HAI による入院に対する分割時系列解析を行い、空間的パターンを評価した。単変量解析および多変量ロジスティック回帰を用いて、ブレイクポイント前後の患者特性および医療処置歴を評価した。
結果
2022 年 3 月に、HAI による入院の上向きの傾きに対する有意な傾きのブレイクポイントが検出された(P < 0.001)。傾向の急上昇は、自己回帰和移動平均モデルで予測された傾向よりも明らかに大きかった。ブレイクポイント後の入院は、心代謝および腎併存疾患による負担の増加、ならびに侵襲的処置への曝露の増加と関連していた。効果量が最大であったのは、慢性腎疾患および血液透析であった。北部および北東部で HAI 率が高かったが、空間的クラスタリングは弱かった(モランの I:0.060 ~ 0.134)。
結論
HAI 率は COVID-19 パンデミック後に増加し、その後も高値を維持したが、これは医療システムの混乱、治療の遅れ、および患者複雑性の増加を反映している可能性がある。将来のリスクを軽減するには、医療のレジリエンスおよび慢性疾患管理の継続性を強化することが不可欠である。
監訳者コメント:
COVID-19 はパンデミック期間中だけでなく、その後の長期的な HAI 負荷に影響することを示した。
