背景

病院のシンクは、医療関連病原体のリザーバーとなる可能性がある。シンクは集中治療室(ICU)における院内アウトブレイクの発生源として同定されているが、アウトブレイク以外の状況におけるそれらの役割は依然として不明である。

目的

ICU 病室内のシンクが、病院感染(HAI)発生率の上昇に関連しているかどうかを検討した。

方法

本解析には、2017 年から 2020 年までのドイツ院内感染症サーベイランスシステム(KISS)の ICU コンポーネントから得られたサーベイランスデータを使用した。2021年 9月から 10月に、参加 ICU すべてにおいて、病室内のシンクの存在について調査した。続いて ICU をシンクなし群(NSG)とシンク群(SG)の 2 群に分けた。主要および副次的評価項目は、全 HAI および緑膿菌(Pseudomonas aeruginosa)に関連する HAI(HAI-PA)とした。

結果

全体で ICU 552施設(NSG N = 80、SG N = 472)からシンク、全 HAI および HAI-PAに関するデータを得た。全 HAI の 1,000 患者日あたりの発生密度は、SG の ICU のほうが高かった(3.97 対 3.2)。HAI-PA の発生密度も SG のほうが高かった(0.43 対 0.34)。すべての病原体に関連する HAI のリスク(発生率比[IRR]= 1.24、95%信頼区間[CI]1.03~1.50)および緑膿菌に関連する下気道感染のリスク(IRR = 1.44、95%CI 1.10~1.90)は、病室内にシンクがある ICU のほうが高かった。交絡因子を補正後、シンクは HAI の独立リスク因子であることがわかった(補正 IRR 1.21、95%CI 1.01~1.45)。

結論

ICU において、病室内のシンクは、患者日あたりの HAI 発生数の増加に関連する。新しい ICU を計画する、または既存の ICU を改修する際には、このことを考慮に入れるべきである。

監訳者コメント


ICU 内の手洗い用シンクがグラム陰性菌の供給源となることが非アウトブレイク時においても示された。また、手洗いシンクを ICU 室内には設置せず、ICU 内でのすべての手指衛生を速乾性消毒剤で実施することでグラム陰性菌の定着の減少が認められており、これらの結果は今後の ICU の病室設計において大きく影響してくることになるが、更なる議論が必要であろう。
