背景

ドイツにおけるクロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)の診断のために標準化プロトコールを用いた縦断的多施設共同研究は存在しない。ドイツの一般的なサーベイランスデータベースの最近の評価では、CDI 発生率の減少傾向が示されている。本稿では、ドイツの大学病院における 2016 年から 2020 年までの CDI の実際の負荷および動向を記述することを目的とした。

方法

本研究はドイツの大学病院 6 施設を対象とする前向き多施設共同研究である。全体のデータ、ならびにサーベイランスの年、医学専門領域、および CDI 重症度により層別化したデータを報告する。重症の CDI のリスク因子を評価するために多変量回帰分析を実施した。

結果

患者 1,436,352 例のうち CDI 症例 3,780 例を登録した。CDI 症例の入院期間(LOS)中央値は 20 日(四分位範囲 11 ~ 37)であったのに対し、全症例の LOS は 4.2 日であった。CDI 患者の院内全死因死亡率は 11.7%(3,780 例中 444 例)、CDI 起因死亡率は 0.4%(3,761 例中 16 例)であった。CDI 再発率は 7.2%と比較的低かった。医療施設で発生した重症医療関連 CDI の発生密度は、2016 年の 10,000 患者・日あたり 2.25 例から、2020 年には 10,000 患者・日あたり 1.49 例と有意に減少した(傾向算定 P = 0.032)。

結論

2013 年から 2014 年の欧州点有病率調査において、ドイツの全 CDI 発生密度は 10,000 患者・日あたり 11.2 例であった(EUCLID)のに対して、本研究では、2020 年の全 CDI 発生密度が 10,000 患者・日あたり 5.6 例と半減した。本稿はドイツの大学病院における CDI の症例分布に関する最新データを示し、CDI の重要な指標に関する国際的な比較データを提供する。
