目的

本研究は、カルバペネム非感受性およびカルバペネム感受性エンテロバクター・クロアカエ(Enterobacter cloacae)複合体の異なるクラスターにおける存在率、耐性率、バイオフィルム形成能および病原性の差について検討することを目的とした。

方法

中国の大学教育病院 1 施設において、カルバペネム非感受性分離株 91 株および同数のカルバペネム感受性分離株、ならびにこれらの臨床情報を収集した。分離株を hsp60 解析に基づいて異なるクラスターに分類した。寒天希釈法を用いて、一般的な抗菌薬における最小発育阻止濃度(MIC)を求めた。クリスタルバイオレットアッセイを用いて、バイオフィルム形成能を測定した。Galleria mellonella* 感染モデルおよび病原性遺伝子の PCR を用いて、病原性を評価した。

結果

分離株は hsp60 解析に基づいて 12 のクラスターに分類した。クラスター VIII は、カルバペネム非感受性分離株の割合が他のクラスターと比べて高かった。同じクラスターでは、カルバペネム非感受性株およびカルバペネム感受性分離株では異なる耐性率が示された。さらに、カルバペネム非感受性分離株が保有する病原性遺伝子はカルバペネム感受性分離株より少なく、クラスター II のカルバペネム非感受性分離株は検出された病原性遺伝子を保有していなかった。カルバペネム非感受性およびカルバペネム感受性分離株の病原性は、G. mellonella 感染モデルを用いた評価では、クラスター I、III、VIII および IX において有意に異なっていた。クラスター VIII におけるカルバペネム非感受性分離株は、他のクラスターと比べて存在率、耐性率、バイオフィルム形成能、および病原性がより高かった。

結論

本研究の結果は、E. cloacae 複合体のサブグループを同定する必要性を示しているとともに、カルバペネム使用に関するより優れた助言と指針を提供している。

中国からの報告。Enterobacter cloacae 複合体の異なるクラスターにおける存在率、耐性率のみならず、バイオフィルム形成能および病原性について報告したのが意義深い。特にクラスター VIII は他のサブグループに比較して優勢であること、カルバペネム非感受性分離株の割合が高いこと、薬剤耐性、バイオフィルム形成能力および病原性が高いことから、注意する必要がありそうだ。
