Risk factors for Klebsiella spp. infection after solid organ transplantation: a multi-centre, case—control study

背景

クレブシエラ(Klebsiella)属菌は、抗菌薬耐性病院感染の主因の1つである。この研究は、移植後最初の 180 日間における Klebsiella 属菌感染のリスク因子である可能性があるものを特定することを目的とした。

方法

多施設共同後向き症例対照研究を、ギリシャとイタリアの 3 つの病院で、2016 年から 2021 年まで実施した。固形臓器移植を受けた 50 歳を超える患者を組み入れた。

結果

合計で 202 例の移植患者(115 例が肝臓、79 例が腎臓、7 例が心臓、4 例が膵臓)をこの研究に組み入れた。そのうち 120 例(59.4%)が、Klebsiella 属菌による感染に少なくとも 1 回罹患した(141 件の感染)。最もよくみられた感染部位は、尿路(48/141、34%)で、最もよくみられた菌種は、肺炎桿菌(Klebsiella pneumoniae)(132/141、93.6%)であった。Klebsiella 属菌の分離株全株のうち、40.3%がカルバペネム耐性であった。臓器のレシピエント全例において、心血管疾患の既往(オッズ比[OR]、2.06、95%信頼区間[CI]、1.14 ~ 3.8、P = 0.019)および移植後集中治療室(ICU)への 3 日以上の入室(OR、2.08、95%CI、1.18 ~ 3.72、P = 0.012)が、Klebsiella 属菌感染と独立に関連していた。肝移植患者では、移植後 ICU への 3 日以上の入室(OR、2.87、95%CI、1.25 ~ 6.78、P = 0.014)がKlebsiella 属菌感染と関連していて、腎移植患者では、心血管疾患の既往が Klebsiella 属菌感染と関連していた(OR、2.84、95%CI、1.12~7.48、P = 0.03)。

結論

50 歳を超える固形臓器移植のレシピエントでは、移植後 ICU への 3 日以上の入室および心血管疾患の既往が Klebsiella 属菌感染のリスクの増加と関連している。
