Epidemiology and outcomes of nosocomial infections in patients undergoing extracorporeal membrane oxygenation support: an eight-year retrospective cohort study

背景

体外式膜型人工肺(ECMO)補助を受けた患者は、医療関連感染症(HAI)に罹患しやすく、このために転帰が悪化する可能性がある。しかし、中南米の施設における疫学データは依然として少ない。

方法

2016 年から 2023 年にかけてブラジルの Hospital Sírio-Libanês で ECMO 補助を受けた全患者を対象に後向きコホート研究を実施した。データは、施設内の ECMO 登録および感染制御サーベイランスシステムから得た。HAI は、米国疾病対策センター(CDC)/全米医療安全ネットワーク(National Healthcare Safety Network)および Brazilian National Health Surveillance Agency の基準に従って定義した。血流感染症(BSI)および人工呼吸器関連肺炎/気管気管支炎(VAP/ VAT)の発生率を算出した。ロジスティック回帰により院内死亡と独立して関連する因子を同定した。

結果

患者 115 例(ECMO 施行 117 件、男性 82%、年齢中央値 61 歳)を解析した。新型コロナウイルス感染症(COVID-19)は、ECMO 施行 77 件を占めた(65.8%)。全発生率は、BSI が 1,000 ECMO 施行日当たり 5.9、VAP/VAT が1,000 ECMO 施行日当たり 7.0であった。非 COVID 19 症例と比較して、COVID-19 患者では ECMO 施行期間が長かったが(中央値 13 日 対 8.5 日)、感染率は同様であった。ECMO 施行期間は長かったが、COVID-19 症例では死亡率は低かった(45.5%)。HAI 患者では、ECMO 施行期間が長かった(中央値 20.5 日 対 10.0 日、P < 0.001)。多変量解析では、高齢(オッズ比[OR]1.07、95%信頼区間[CI]1.04 ~ 1.11)およびHAI (OR 4.01、95%CI 1.23 ~ 15.12)が独立して死亡と関連していた一方、COVID-19 の診断には死亡に対する保護作用が認められた(OR 0.22、95%CI 0.05 ~ 0.75)。

結論

ブラジルのこの ECMO センターでは、世界の大部分の報告と比較して HAI の発生率が低く、このことから感染予防が効果的であったことが示唆される。しかし、ECMO 施行期間は依然として主要な変更可能なリスク因子であり、HAI は独立して死亡率上昇と関連した。継続的サーベイランス、予防バンドル、および抗菌薬適正使用支援は、ECMO 施行の転帰を改善する上で極めて重要である。

監訳者コメント

ECMO 患者では、長期間の人工呼吸管理、複数の血管内デバイス、回路操作などが重なり、感染リスクが時間とともに増大する。本研究は、標準化したサーベイランスと予防バンドル、継続的な監査により感染率を低く保てる可能性を示す一方、医療関連感染が死亡と強く関連することを示した。ICN・ICT は ECMO チームに参画し、カニューレ・中心ライン管理、口腔ケア、体位変換時の手技、培養採取、抗菌薬使用を横断的に評価する必要がある。感染予防を単独の手技としてではなく、毎日の ECMO 離脱評価や抗菌薬適正使用を含む多職種診療の一部として組み込む視点が求められる。
