ベトナムにおけるカルバペネム耐性グラム陰性菌感染症の負荷:全国病院調査★★

緒言
世界保健機関の細菌性病原体優先順位リスト 2024 年版(Bacterial Pathogen Priority List 2024)では、カルバペネム耐性腸内細菌目細菌(CRE)、カルバペネム耐性アシネトバクター・バウマニ(CRAB)およびカルバペネム耐性緑膿菌(CRPA)を含むカルバペネム耐性グラム陰性菌(CRGNB)が、その病原性、耐性、伝播および限られた治療選択肢のために、最優先として強調されている。
目的
この全国病院調査の目的は、ベトナム全体の CRGNB 感染症の負荷を評価し、微生物学的検査室の対応能力を判定することであった。
方法
オンライン調査をベトナムの中央病院および 63 の省の保健当局に配布し、そこから地域および民間病院に転送した。
結果
全体で本調査には 324 施設の病院が参加した。内訳は、中央病院 20 施設、省立病院 190施設、地域病院 106 施設、民間病院 8 施設であった。半数において、細菌培養および感受性検査の微生物学的対応能力が報告された。報告されたグラム陰性分離株 57,667株中、17,417 株(30.2%)が CRGNB であり、内訳はカルバペネム耐性肺炎桿菌(CRE 37%)、カルバペネム耐性腸内細菌目細菌(CRE 11%)、カルバペネム耐性A. baumannii (CRAB 64%)およびカルバペネム耐性緑膿菌(CRPA 39%)であった。CRE 敗血症の治療に用いられたのは、セファロスポリン系、アミノグリコシド系、カルバペネム系、フルオロキノロン系およびコリスチンなどであった。CRE 感染例では、カルバペネム感性腸内細菌目細菌感染例と比較して、粗死亡率が高く(31.7% 対 20.2%、P < 0.001)、入院期間が長く(10.4日 対 8.9日、P < 0.001)、費用が高かった(1,025 対 773、P < 0.001)。報告された感染予防・制御(IPC)介入は、CRE サーベイランス(41%)、隔離(36%)、点有病率調査(15%)、コホートケア(7%)などであった。IPC の財源は、一般健康保険(40%)、病院資金(32%)などであった。
結論
CRGNB は、罹患率、死亡率および経済的影響という点で重大な負荷をもたらす。微生物学的検査のインフラの強化、CRGNB スクリーニングの改善および IPC 措置の増強が急務である。

ベトナムにおけるカルバペネム耐性グラム陰性菌(CRGNB)の、全国病院調査の報告である。報告されたグラム陰性分離株のうち、3 割がカルバペネム耐性グラム陰性菌(CRGNB)であったと報告している。カルバペネム耐性腸内細菌目細菌(CRE)感染症の感染例は、カルバペネム感性腸内細菌目細菌感染例と比較して粗死亡率が高く、入院期間が長く、医療費が高かった。
