目的

この研究は、2 施設のデータに基づいていて、緑膿菌(Pseudomonas aeruginosa)による血流感染症(BSI)患者におけるカルバペネム耐性(CRPA)および多剤耐性(MDR)のリスク因子を深く解析することと、独立したリスク因子としての肺感染症の役割を探ることが目的であった。

方法

これは、2 つの 3 次病院で実施された後向きコホート研究であった。2014 年から 2024 年の緑膿菌による BSI 患者 499 例のデータを後向きに組み入れた。Boruta アルゴリズムを使って CRPA/MDR の予測因子をスクリーニングし、続いて、回帰モデルによって、独立したリスク因子としての肺感染について分析した。結果の頑健性は、多施設一貫性試験、サブグループ解析、感度分析で検証した。

結果

CRPA の発生率は 26.3%、MDRの発生率は 40.9%であった。ロジスティック回帰分析では、肺感染が CRPA と MDR の中核となる予測因子であることが示された。肺感染症は、CRPA(オッズ比[OR]= 2.59、P < 0.001)と MDR(OR = 1.82、P < 0.001)のリスクを有意に増加させ、機械換気と相乗的に薬剤耐性を悪化させた(CRPA の OR = 2.98、MDR の OR = 2.35)。多施設一貫性試験(交互作用の P > 0.10)とサブグループ解析によって、結果の安定性が確認された。

結論

肺感染症は、緑膿菌による BSI のカルバペネム耐性および多剤耐性の重要なリスク因子である。この結果は、施設間一貫性試験、サブグループ解析、感度分析の後でも安定していた。
