背景

中心ライン関連血流感染症(CLABSI)は、かなりの病的状態および死亡と関連している。CLABSI 発生率の長期的動向と原因微生物の種類に関するデータは限られている。

目的

CLABSI 発生率、中心ラインの使用、および原因微生物における動向を記述すること。

方法

2016 年 1 月から 2023 年 12 月の間に、スイスの 3 次病院における CLABSI について前向きに収集されたデータを解析した。CLABAI の発生密度、カテーテル使用および原因微生物における長期的変化を検討した。

結果

CLABSI 計 707 件が観察され、これは発生密度で 1,000 カテーテル日当たり CLABSI 1.69 件(95% 信頼区間 1.56 ~ 1.81)に相当した。1,000 カテーテル日当たり CLABSI 発生密度に有意な傾向は認められなかったが(z = 0.86、P = 0.391)、カテーテル使用率には有意な上昇が認められた(z = 8.88、P < 0.001)。コアグラーゼ陰性ブドウ球菌(N = 207、23.4%)および腸球菌属(Enterococcus spp.)(N = 134、15.2%;エンテロコッカス・フェシウム[Enterococcus faecium]、N = 94、10.6%;エンテロコッカス・フェカーリス[Enterococcus faecalis]、N = 37、4.1%;その他の腸球菌属、N = 3、0.34%)は、最も多く認められた原因微生物であった。長期的にみると、腸球菌属の割合(z = 3.4、P < 0.001)(E. faeciumの増加による[z = 3.2、P = 0.001]、および酵母菌(z = 2.3、P = 0.020)が増加した一方、コアグラーゼ陰性ブドウ球菌は減少した(z = -6.1、P < 0.001)

結論

前向き CLABSI サーベイランスにより、CLABSIの 1,000 カテーテル日当たり発生密度は一定であったが、原因微生物には経時的に有意な変化が認められることが示された。

監訳者コメント:

スイスの単施設における 8 年間の CLABSI サーベイランスの結果であるが、腸球菌属、なかでもE. faeciumの増加は入院期間の延長と関連があり、同時にこれらの長期入院患者へのセファロスポリン系抗菌薬の使用頻度の増加も関係している可能性がある。本菌による血流感染発生時にはこれらの薬剤は効果が期待できない。さらに ICU 入室患者へのクロルヘキシジン清拭も 2017 年には導入されている。米国 SHEA/IDSA/APIC の CLABSI 血流感染防止 2022 年版を再確認しておきたい。
