背景

最近の研究で、少なくとも 6 時間継続した通常の麻酔の実施後にシリンジチップのかなりの汚染が確認された。

目的

高伝搬性で、病原性が高まっている黄色ブドウ球菌(Staphylococcus aureus)シークエンス型 5(黄色ブドウ球菌 ST5)株による汚染後のシリンジチップの臨床的に重要な消毒法の相対的な有効性を評価した。この黄色ブドウ球菌 ST5 の特徴は、バイオフィルム形成の強度が増強していて、乾燥耐性が強くなっていることと関連している。

方法

黄色ブドウ球菌 ST5 で汚染されたシリンジチップ(N = 40)を、70%のイソプロピルアルコールパッドで乾燥時間が 10 または 60 秒、スクラブアルコール消毒キャップで留置時間が 10 または 60 秒、またはスクラブなしのアルコール消毒キャップで留置時間が 60 秒に無作為化した。主要転帰は、残存した 24 時間コロニー形成単位(cfu)が> 10とした。

結果

スクラブ消毒キャップは、アルコールパッドよりも有効であった(スクラブキャップ[留置時間 10 または 60 秒]の 25%[12/48]が<10 cfuに対して、アルコールパッド[乾燥時間 10 または 60 秒]の 0%[0/48]が< 10 cfu、Holm–Sidak による補正後のP = 0.0016)。スクラブ消毒キャップは、スクラブなしのアルコール消毒キャップよりも有効であった(スクラブアルコールキャップ[留置時間 10 または 60 秒]の 25%[12/48]が<10 cfuに対して、スクラブなしのアルコールキャップ[留置時間 60 秒]は 2%[1/48]、補正後のP =0.0087)。

結論

スクラブアルコールキャップは、病原性が高い黄色ブドウ球菌 ST5 で汚染されたシリンジチップの微生物を減少させることにおいて、アルコールパッドや、スクラブなし消毒キャップよりも有効である。

監訳者コメント

針なしコネクタは針刺し切創を減らすことができるが、注射器の先端、注入ポート、注入経路を細菌汚染にさらす可能性があり、麻酔作業領域のバスキュラーアクセス感染症の原因となりうる。本研究は、細菌のバイオフィルムを機械的に破壊するスクラブ機構を備えた消毒キャップが、スクラブ機構のないアルコールパッドやアルコール消毒キャップよりも、黄色ブドウ球菌(ST5 株)の減少に大きな効果を示している。
