バンコマイシン耐性エンテロコッカス・フェシウム（Enterococcus faecium）の持続するアウトブレイクの解析には、適応させた診断アルゴリズムが必要であることが明らかになった★

目的
研究対象の施設では、vanA および／または vanB を持つ分離株を含む様々なバンコマイシン耐性エンテロコッカス・フェシウム（Enterococcus faecium）（VREfm）によるアウトブレイクがあり、その能力が試された。驚くべきことに、一晩濃縮した検体を vanA と vanB のリアルタイムポリメラーゼ連鎖反応（PCR）を使ってスクリーニングしたところ、非常に低いサイクル閾値で vanB の結果が陽性となった一方で、VREfm 特異的な濃縮培養は陰性のままであった。この論文では、診断アルゴリズムを適応させるに至った診断結果の解析について説明する。
方法
vanA および vanB のスクリーニング PCR と、VREfm 特異的培養（Brilliance VRE）の結果を収集し、特定された VREfm 分離株の遺伝子型のデータと組み合わせた。アウトブレイク中、もう1つの VREfm 特異的培地（CHROMagar VRE）が導入され、その結果を、Brilliance VRE 寒天で得られた結果と比較した。
結果
35 例の患者が VREfm の保菌者として特定され、4 つの異なる株が同定された。すなわち、vanA（STnew-CT7088）および／または vanB（ST80-CT1065、ST117-CT7117、ST117-CT7118）であった。PCR 結果を補完する培養と遺伝子型決定を行ったところ、Brilliance VRE 寒天による培養が、特定された vanB ST117 分離株の検出に、バンコマイシンの最小発育阻止濃度に関係なく、十分ではないことが明らかになった。それとは対照的に、CHROMagar VRE は、vanB ST117 分離株とその他の試験した分離株を正しく検出できた。
結論
VREfm 特異的培地による vanB ST117 分離株の検出は不十分であった。これが、VREfm が気付かれずに広がっていることの一因である可能性がある。このために、CHROMagar VRE がアウトブレイク中に評価を受け、その後、通常の診断で実施されるようになり、Brilliance VRE 寒天に取って代わった。

Brilliance VRE と CHROMagar VRE でバンコマイシン耐性 Enterococcus faecium の検出は異なっていた。
