背景

手術部位感染症(SSI)は大腸手術後に多くみられるが、ほとんどの病院は自院の SSI 発生率を認識していない。SSI のほぼ半数が退院後に発生しており、発生率の正確な測定には退院後のサーベイランスが必要である。周術期ケアバンドルは、SSI 発生率を低減することが知られている。PreciSSion は、西イングランドの病院 7 施設間の共同事業である。

 

目的

患者報告による30 日転帰指標を用いて、待機的大腸手術後の信頼できる SSI 発生率測定を確立すること、ならびに地域病院で SSI 発生率を低減することが既に実証されている、エビデンスに基づく 4 要素からなるケアバンドルを実施すること。同バンドルは、2%クロルヘキシジンによる皮膚前処置、4 時間後の抗菌薬再投与、二重リング創傷保護具の使用、および抗菌縫合糸を用いた腹壁閉鎖からなっている。

 

方法

患者報告による30 日 SSI 発生率を Public Health England 質問票を用いて確定し、回答率を記録した。ベースラインの SSI 発生率を 2019 年 11 月から 2020 年 5 月まで測定し、ケアバンドル実施後の 2021 年 3 月まで継続した。バンドルの遵守率も測定した。

 

結果

質問票への平均回答率は 81%、バンドルの各要素の平均遵守率はそれぞれ 92%、96%、79%および 85%であった。ベースラインの SSI 発生率は 8 ~ 30%であった。 7 施設のうち 6 施設で SSI 発生率が低下し、地域の平均 SSI 発生率は 18%(1,447 例)から 9.5%(1,247 例)へとほぼ半減した。

 

結論

単一病院で作成されたケアバンドルは他の病院でも採用可能であり、待機的大腸手術後の SSI 発生率における 50%という低減は、他院でも 18 か月以内に再現可能である。
