肺炎桿菌(Klebsiella pneumoniae)における消毒薬耐性の発達★

背景
消毒薬は重要な感染制御手段であり、医療、食品製造、家庭環境など、さまざまな場面で世界的に信頼されている。しかし、細菌は乾燥表面のバイオフィルムに保護されていたり、消毒薬が効果的に使用されなかったりする場合に生残することが示されている。このことは細菌が様々な消毒薬に対して低レベルの耐性を獲得する機会を提供することとなる。抗菌薬以外の製剤に対する細菌の適応能力は、しばしば見過ごされている。
目的
臨床的に重要な肺炎桿菌(Klebsiella pneumoniae)が、世界中で日常的に使用されている一般的な消毒薬に適応する能力と準備態勢について報告することで、抗菌薬耐性の見落とされがちな側面について、大いに必要な知見を得る。
方法
本研究では、Klebsiella pneumoniae NDM-1 株(NCTC 13443)が、一般的な化学消毒薬である塩化ベンザルコニウム、ジデシルジメチルアンモニウム塩化物、ポリヘキサメチレンビグアニド、クロロクレゾール、ブロノポールについて適応する能力を、連続曝露方法を用いて調査した。
結果
長期適応後、肺炎桿菌はすべての対象消毒薬に対して耐性を獲得し、未処理の親株と比較して、最小発育阻止濃度(MIC)は 30%から 413%増加した。消毒薬の交差耐性の特性評価では、交差耐性が発生する場合もある一方で、多くの適応サンプルは 2 回目の消毒剤処理に対してより感受性が高くなった。これは、おそらく環境適応にかかる代償のためであろう。観察された交差耐性や付随的感受性は、すべての消毒薬耐性サンプル間で必ずしも相互作用していなかった。
結論
この結果は、耐性発現には消毒薬曝露の順序が重要であることを示唆した。これは、消毒薬の洗浄ルーチンに重大な影響を及ぼすものであり、消毒薬の作用機序が類似している場合であっても、耐性の基礎となる機序にばらつきがあるためと考えられる。
監訳者コメント :
本研究で消毒薬の交差耐性や曝露順序の重要性といった新しい知見は、国内で消毒薬を使用する際の参考になるであろう。特に塩化ベンザルコニウムとジデシルジメチルアンモニウムクロライドのような同じ MOA を介して作用する消毒薬間で交差耐性のばらつきが生じたこと、ブロノポールおよびポリヘキサメチレンビグアニド耐性の交差耐性プロファイルが生物学的複製間でばらつきが見られたことは未解決問題である。臨床現場では今後消毒薬を使用する際の選択や順序についてどのぐらい影響がでるかを検証することが必要と思われる。
