大きな大学病院の 3 つの病棟における抗菌薬の処方の適切性に対する教育的な抗菌薬適正使用支援プログラムの効果:分割時系列解析★

背景
抗菌薬適正使用支援(AMS)プログラムは、抗菌薬の処方の質を高め、抗菌薬の使用を減らし、多剤耐性の病原体と戦うことを目的としている。しかし、様々な臨床状況に対して、AMS によるどのような介入が最適であるかは、依然として、はっきりしていない。これまでの研究は、処方の適切性よりも、抗菌薬の消費量に焦点を当てたものが多かった。
目的
この研究は、1,500 床の大学病院の 3 つの病棟において、教育に基づく AMS による介入が、抗菌薬の処方の適切性に与える影響を評価することを目的とした。
方法
2018 年 6 月から2019 年 10 月まで、ローマにある我々の 1,500 床の大学病院の 3 つの病棟を対象に、教育プログラムの効果を試験するための後向き介入分割時系列研究を実施した。介入は、3 か月(2018 年 12 月から 2019 年 2 月)に亘って開催された 6 回の教育的会合から成っていた。主要転帰は、抗菌薬の処方の適切性とし、院内での生存を副次的転帰とした。
結果
評価した 609 件の抗菌薬の処方では、プログラムによって、1 つの病棟で、不適切な処方が有意にそして持続的に減少したが(レベルの変化、-18.15%、P < 0.01、傾向の変化、-3.21%、P = 0.01)、それ以外の 2 つの病棟では有意な減少が示されず、3 つの病棟全体で見た場合も有意な減少が示されなかった。
結論
構造や組織の特徴が似通っていた 3 つの病棟における抗菌薬の適切性に関して、同じ教育的な AMS プログラムがばらついた結果をもたらした。抗菌薬の処方の適切性に対する教育プログラムの影響をより理解するためには、より規模が大きく、より目的に合った質の高い AMS 介入試験が必要である。
監訳者コメント :
抗菌薬適正使用に関して、同じ教育プログラムを実施しても 3 つの病棟で異なった結果がでている。抗菌薬適正使用プログラムは、対象とする施設やユニットの状況に合わせた個別のプログラムを作成する必要性が本研究から示唆される。感染症診断への基本的アプローチは同じでも、治療における抗菌薬処方は、ユニット内の患者の疾患特性の違い、処方決定のプロセス、ユニットの組織文化や協力体制の違い、などに影響されるため、これらを十分に考慮しながら、進めてゆくことが必要である。
