Efficacy of standard precautions versus proactive containment for nosocomial Candida auris outbreaks: a systematic review and meta-analysis

緒言

カンジダ・オーリス(Candida aurisC. auris])の環境レジリエンスと外因性伝播ダイナミクスは、院内感染の制御に持続的な課題をもたらし、現在の国際ガイドラインは発生時の化学消毒に大きく依存する。そこで本研究では、これらの標準的なガイドラインに基づく感染プロトコールの運用有効性を数学的に定量し、先進的な事前対応型の封じ込め策と比較した。

方法

2016 年から 2025 年までの PubMed および Scopus を用いて、Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)に準拠したシステマティックレビューおよびメタアナリシスを実施した。7 件の国際公衆衛生ガイドラインのコンセンサス指示を、23 群の臨床アウトブレイクコホートから抽出した経験的データと構造的に比較した。Mantel-Haenszel 法を用いた変量効果モデリングにより、失敗率および相対的リスク比(RR)の統合値を推定した。

結果

ガイドラインの指示による標準的接触予防策および発生時の化学消毒を、単独の戦略として使用した場合、運用失敗および再汚染率の統合値は 0.708(95%信頼区間[CI] 0.559 ~ 0.830)となった。逆に、事前対応型の封じ込め策の実施は、伝播リスクの79%低下に関連した(RR 0.21、95%CI 0.12 ~ 0.37、P < 0.001)。頻繁に取り扱われる再利用可能な臨床媒介物の対象を絞った使用中止によって、最も高いアウトブレイク終結効果が達成され(RR 0.08、95%CI 0.02 ~ 0.32)、入院時の迅速分子スクリーニングによって補われた(RR 0.19、95%CI 0.04 ~ 0.97)。

結論

標準的な事後対応型の化学消毒と接触予防策は、C. auris の院内伝播の停止において運用失敗率が高い。感染予防プロトコールを最適化するには、事前対応型の介入への移行が必要であり、特に診断の迅速化および再利用可能な臨床媒介物の厳密な管理に優先的に取り組まなければならない。
