目的
内科集中治療室(ICU)における抗菌フィルターの取り付けにもかかわらず、単一遺伝子型(シークエンス型[ST]299)が定着した給水設備から発生した緑膿菌(Pseudomonas aeruginosa)による水系汚染の残存リスクを評価すること。
方法
ICU 開設から最初の 19 か月間に、給水設備の汚染について、フィルターの上流で水を採取して月一回評価した。フィルターの効果を評価するために、下流での水サンプルも採取した。患者から緑膿菌分離株を採取し、水系緑膿菌 ST299 との比較を、multiplex rep-ポリメラーゼ連鎖反応(PCR)、パルスフィールド・ゲル電気泳動(PFGE)、全ゲノムシークエンシングにより実施した。緑膿菌の他の遺伝子型による交差伝播の事象も評価した。
結果
濾過水の 449 サンプルのうち 1.3%で播種材料中の緑膿菌が陽性で、蛇口によって 1 ~ 104 コロニー形成単位/100 mLと全体的にばらつきがあった。水生緑膿菌のすべての分離株が ST299 に属し、一塩基多型(SNP)は 2 未満であった。患者 122 例の臨床分離株 278 株のうち、患者 5 例の 10 株は、multiplex-rep PCR と PFGE の両方で、水生 ST299 クローンと同一のプロファイルを示し、SNP の平均値は 5 未満により異なり、患者の 4.09%の緑膿菌による汚染源として水道設備がリザーバであることが確認された。緑膿菌の他の遺伝子型による交差伝播事象は、患者の 1.75%における汚染の原因であった。
考察/結論
水道設備が非常に汚染されている場合、患者を水系病原体から防御するのに抗菌フィルターは十分ではない。緑膿菌感染症リスクのある患者を受け入れる病棟では、すべての水使用場所にフィルターが取り付けられていても、濾過水の微生物学的調査が必要と考えられる。

院内の特定部署ですべての水使用場所にフィルターが取り付けられていても、濾過水の微生物学的調査が必要な場合があることを示す警鐘的事例である。感染ルートをたどるとともに、その周囲に汚染源が存在する可能性も十分に検証しなければならないだろう。
