Identifying potential predictors of the risk of surgical site infection following cardiac surgery: a scoping review

目的

本スコーピングレビューは、成人の心臓手術における手術部位感染症（SSI）のリスク予測モデルおよび術前予測因子を特定することを目的に実施した。SSI のリスクが高い患者を特定するリスク予測モデルの今後の開発の土台となり、したがって国の SSI 予防戦略の指針となるような、新たな予測因子の特定に特に重点を置いた。

方法

SSI の術前予測因子に関する既存の研究によるエビデンスを系統的に特定し、解析するためのスコーピングレビューを 2 段階で実施した。第 1 段階では、心臓手術における SSI の予測モデリング研究を検討した。第 2 段階では、新たな心臓 SSI 予測因子に関する一次研究およびシステマティックレビューを特定した。

結果

検索により、7,887 報の独自報告を特定した。アブストラクトのスクリーニングにより 7,154 報を除外し、733 報をフルテキストの評価に選択した。第 1 段階に組み入れた 29 件の研究（30報）において、67項目の異なる術前予測因子を含む 52 種類の SSI リスク予測モデルの開発（N ＝ 14）、検証（N ＝ 13）、開発と検証の両方（N ＝ 2）が報告されていた。残りの 703 報を第 2 段階で再評価したところ、49 件の研究が選択基準を満たし、これまでにモデルにおいて評価されていない新たな術前予測因子 56 項目が特定された。

結論

本レビューでは、心臓手術後の SSI リスクの術前予測因子 123 項目が特定され、そのうち56 項目はそれまでの心臓 SSI リスク予測モデルの開発に含まれていなかった。これらの予測因子候補は、リスク予測スコアの今後の開発に有用な資料となり、他の外科専門分野における SSI リスクの予測に関連する可能性がある。
