病院感染予防・制御のための AI:現場実装、インパクト、モデル開発以降の空白★

背景
人工知能(AI)と機械学習(ML)は、自動サーベイランス、早期警告、意思決定支援を通して病院の感染予防・制御(IPC)を強化する可能性があるが、エビデンスの基盤は断片的であり、後向きのモデル開発に限定されている場合が多い。
方法
Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020(PRISMA 2020)に準拠したシステマティックレビューを実施し、急性期ケア病院の IPC における AI/ML の研究を、ワークフローに組み込まれた実装済みのシステムと未実装の開発/検証研究に分けて統合した。2026 年 1 月 17 日に、PubMed/MEDLINE、Scopus、Web of Science Core Collection および Cochrane CENTRAL で検索を行った。2 名のレビュアーが独立して記録をスクリーニングし、フルテキストを評価した。
結果
特定された 7,089 件の記録のうち、重複する 2,061 件を削除した。5,028件のタイトルとアブストラクトをスクリーニングし、177 件のフルテキストを評価し、59件の研究を組み入れた。59 件中 9 件のみが現場実装を報告した(パイロット 6 件、ルーチン 3 件)一方、59 件中 50 件は開発/検証のみにとどまった。従来型 ML が圧倒的に多く(42/59 件)、続いて自然言語処理(8/59 件)、深層学習(5/59 件)、ルールベースまたはエキスパートシステム(4/59 件)であった。実装済みの研究では、標準化された感染アウトカムよりも、プロセスおよび運用アウトカムが報告された頻度が高く、不必要な隔離やアラート疲れなどの予期せぬ結果を定量したものはほとんどなかった。特に移植性や、対照群を置かない前後評価における交絡に関して、バイアスのリスクおよび適用可能性の懸念が多く認められた。
結論
病院 IPC のための AI/ML は有望であるが、日常診療への移転は依然として限定的である。より安全な採用のためには、実装に焦点を合わせた研究デザイン、標準化された IPC 関連アウトカム、配置後の継続的なモニタリングによるライフサイクル管理が必要である。
