重症患者におけるインフルエンザ関連の肺アスペルギルス症の臨床的特徴と転帰:システマティックレビューとメタアナリシス★

Clinical characteristics and outcomes of influenza-associated pulmonary aspergillosis among critically ill patients: a systematic review and meta-analysis

背景

侵襲性肺アスペルギルス症は、インフルエンザの合併症の 1 つと次第に見なされつつあり、「インフルエンザ関連肺アスペルギルス症」(IAPA)と名付けられている。

 

目的

IAPA を有するまたは有さない重症インフルエンザ患者の罹患率と死亡率を評価すること。

 

方法

PubMed、Cochrane Library、Scopus および Embase データベースで、IAPA を有する重症インフルエンザ患者の比較データを含む研究を検索した。主要転帰は、全死因院内死亡率および集中治療室(ICU)死亡率とした。副次的転帰は、臨床的特徴、侵襲的人工呼吸の期間、ICU 滞在期間および入院期間、昇圧剤、腎代替療法および体外式膜型人工肺(ECMO)の必要性とした。

 

結果

重症インフルエンザ患者 853 例において、IAPA の発生率は 28.8%であり、全死亡率は 33.4%であった。年齢および併存疾患に差は認められなかった。IAPA を有する患者は大部分が男性であり、慢性的なステロイドの投与を受けていた。IAPA を有する患者のほうが有さない患者よりも、院内死亡率(49.2%対 27.0%、P = 0.002)および ICU 死亡率(46.8%対 20.8%、P < 0.001)が高かった。IAPA 患者の大部分が侵襲的人工呼吸を必要とし、侵襲的人工呼吸の期間(平均 17.3 日対 10.5 日、P < 0.001)、ICU 滞在期間(平均 26.8 日 対 12.8 日、P = 0.001)および入院期間(平均 38.7 日対 27.0 日、P = 0.003)が延長した。IAPA 患者は、IAPA を有さない患者と比較して、疾患重症度がより高く、昇圧剤(76.4%対 57.9%、P < 0.001)、腎代替療法(45.7%対 19.1%、P < 0.001)および ECMO(25.9%対 12.8%、P = 0.004)の支援を必要とする可能性が有意に高かった。

 

結論

重症患者における IAPA の診断は、より高い罹患率および死亡率に関連している。より良い診断・治療戦略を決定するため、早期発見とさらなる研究が必要である。

監訳者コメント

侵襲性肺アスペルギルス症がインフルエンザの合併症として 28.8%もあるのには驚かされる(IAPA)。我が国でも症例報告はあるが、大規模な疫学調査報告は見当たらない。今後の検討が必要であろう。
