背景

オーストラリアにおける医療関連感染症のサーベイランスはばらつきがあり、リソースを集中させ、持続的でなく、限られた情報しか得られない。従来の医療関連感染症サーベイランスは、時間がかかり、医師間の一致度にばらつきがあることがこれまでに示されている。

目的

2 つの手法、すなわち半自動化アルゴリズムおよびコード化データと、従来の手術部位感染(SSI)サーベイランス法を比較すること。

方法

この後向き多施設コホート研究には、都市圏の大規模病院 2 施設において 2 年間にわたり股関節形成術(HPRO)または膝関節形成術(KPRO)および冠動脈バイパス術(CABG)を受けたすべての患者を組み入れた。感染予防・制御(IPC)チームからルーチンの SSI データを得て、以前に開発したアルゴリズムをすべての患者記録に適用し、SSI を有するとして分類された患者についてオーストラリアで用いられている ICD-10-AM のデータを調べた。

結果

全体で、それぞれ HPRO、KPRO および CABG を受けた患者 1,447 例、1,416 例および 1,026 例を組み入れた。アルゴリズムにより生成された感度の最高値は、HPRO における深部 SSI または臓器・体腔 SSI について 0.87(95%信頼区間[CI]0.66 ~ 0.96)、CABG について 0.86(95%CI 0.64 ~ 0.96)、および HPRO における全 SSI について 0.77(95%CI 0.57 ~ 89)であった。最も低い感度は CABG における深部 SSI または臓器・体腔 SSI のコードについて 0.03(95%CI 0.00 ~ 0.21)であった。アルゴリズムにより生成された陽性的中率(PPV)の最高値は、HPRO における深部 SSI または臓器・体腔 SSI について 0.97(95%CI 0.77 ~ 0.99)、CABG における深部 SSI または臓器・体腔 SSI について 0.97(95%CI 0.76 ~ 0.99)、HPRO における深部 SSI または臓器・体腔 SSI のコードについて 0.9(95%CI 0.66 ~ 0.99)であった。アルゴリズムとコード化データのいずれによっても、レビューすべき医療記録の件数が大幅に減る結果となった。

結論

SSI サーベイランスを強化するためのアルゴリズムの適用により、SSI の存在を確認するために IPC チームがレビューすべき患者記録を同定する上で高い正確度が得られることが示された。コード化データ単独では SSI の同定に用いるべきでない。

監訳者コメント

SSI サーベイランスは HAI サーベイランスの中で最も手間のかかるサーベイランスのひとつである。従来の方法に比べ、半自動化アルゴリズムの適用はコード化データの使用よりも見逃しは少なく、感染管理者の手間を大幅に減らすことができる。半自動化アルゴリズムを適用した SSI サーベイランスはコストパフォーマンスが高い方法になる可能性がある。
