Environmental reservoirs of methicillin-resistant Staphylococcus aureus in isolation rooms: correlation with patient isolates and implications for hospital hygiene

メチシリン耐性黄色ブドウ球菌（MRSA）の蔓延を管理・予防するための戦略には、スクリーニングによる陽性患者の早期発見、患者の隔離、手指消毒、鼻腔と皮膚のコンタミネーション除去、臨床現場の適切な清掃とコンタミネーション除去などが含まれる。しかし、各国の多数のガイドラインには環境のコンタミネーション除去法に関する記述が少ないが、その一因として、MRSAの蔓延において環境が果たす役割が明らかではないことが挙げられる。著者らは、病院のプロトコールに従って日常の定期的な清掃を継続する一方、MRSA患者25例の隔離室の環境において、水平な環境表面、エアーサンプラーを用いた空気の検体、ならびに静置培地による検体採取を行い、4週間にわたって前向きに調査した。その後、菌株の類似性を評価するために、20の患者分離株およびそれらに対応する環境分離株（35株）のタイピングを行ったところ、検体中のMRSA陽性検出率は、環境表面の検体269/502（53.6％）、空気の検体70/250（28％）、静置培地の検体102/251（40.6％）と高率であった。ベッドおよびマットレスから採取した表面からの検体の半数以上がMRSA陽性であった。患者14例（70％）において、患者とその患者の環境から同一菌株または密接に関連する菌株が回収されたことから、隔離室の環境汚染の可能性が示唆されたが、このことはMRSAの地域的流行につながる可能性がある。MRSAおよびその他の病院感染病原体を根絶するための新しい技術を検討し、清掃とコンタミネーション除去への現行の取り組みをさらに効果的かつ厳重に行うことが必要である。
