Implementation considerations for integrated face and respiratory protection: a qualitative study

背景

呼吸器伝染病に対する感染予防制御策として、革新的な形態の個人防護具（PPE）を導入することは複雑であり、対処すべきいくつかの要素がある。

目的

臨床における一体型顔面・呼吸器防護具の導入について検討する。

方法

本研究は、医療従事者 87 名を対象に、従来の PPE または電動ファン付き呼吸用保護具（PAPR）と軽量 PAPR（L-PAPR）とを比較した、多施設共同定性的研究であった。実装研究のための統合フレームワーク（CFIR）に基づき、半構造化面接を実施した。

結果

L-PAPR の導入に関する知見が見出された。利点は、防護感の向上、心地よい換気、医療従事者と患者の両者にとって好ましい視界、バイザー内部の曇りなし、動作の制限なし、容易な消毒過程などである。使いやすさを向上させるために、いくつかの障壁に対処すべきであり、顔面圧迫感の低減、眼鏡や矯正レンズの収まりの改善、器具組立てのステップ数の減少、器具の保管、充電および消毒のためのインフラの提供、組立て、装着および脱装に関する医療従事者の訓練、導入の費用便益などが挙げられる。

結論

L-PAPR は、全体として多数の参加者によって利点があると受け止められたことから、呼吸器伝染病のアウトブレイク時に医療従事者を防護するために導入すべき PPE の選択肢になりうると考えられる。

L-PAPR はまだ利用率は低いが、機能面からこれまでの呼吸器防護のための PPE として、積極的に取り入れるべきであろう。
