深部皮膚における細菌のリザーバは、手術部位感染症および生体材料関連感染症の潜在的発生源である★★

背景
手術部位感染症および生体材料関連感染症の原因は、依然としてはっきりしない。創部を汚染する微生物は、手術室の空気、手術チーム、または患者の皮膚に由来することがある。患者の皮膚は、手術前に消毒されるが、皮膚の深部(汗腺、皮脂腺など)の細菌には到達しない可能性がある。
方法
2020 年 5 月 から 2021 年 2 月にかけて、手術部位感染症と生体材料関連感染症の原因を検討するために、予備的なコホート研究を実施した。切開により皮膚から創部を切除した場合、皮膚の微生物叢がコロニー形成するか否か調査するために、外傷手術を受けた患者 99 例を対象に、皮膚の消毒前後に、最初の切開後のメスブレードと直接創部から皮膚由来の好気性菌および嫌気性菌を分離し、定量化し、同定した。
結果
クロルヘキシジンによる消毒前に、患者の 99%が培養陽性であった。このうち 40%は消毒後も培養陽性で、うち 54%は皮膚切開後に創傷培養が陽性であった。消毒後に培養陰性であった患者の 20%は、皮膚切開後の創部培養が陽性であった。表皮ブドウ球菌(Staphylococcus epidermidis)とキューティバクテリウム・アクネス(Cutibacterium acnes)は、最も多く培養された細菌種であった。症例の 9%で、創部から 100 超の細菌コロニーが培養され、これは生体材料関連感染症を引き起こす可能性がある。
結論
皮膚に棲息し、消毒薬により死滅しない細菌は、切開時に手術創に侵入し、結果として、生体材料関連感染症を起こしうる汚染につながる。
監訳者コメント:
生体材料を使用した手術における術後感染症の原因は、手術室の空気中に浮遊する細菌あるいは皮膚上の細菌によるとの議論がこれまでされてきた。本論文からは前者よりも後者である可能性が極めて高く、術前に皮膚消毒を実施しても、毛根や皮脂腺・汗腺の皮膚深部に潜んでいる細菌を確実に殺菌消毒することはできない。切開とともに創部内に入り込んだ細菌は創内で増殖し感染症を引き起こす可能性が高い。検出された菌種はこれらの感染を引き起こす原因となりうる。これらの混入を防ぐために、研究結果から皮膚切開直後の創内を創部治癒を妨げない消毒剤で洗浄する「二次消毒」を提案している。これまで皮膚深部への浸透性を高めるために消毒剤にユーカリ油の添加、あるいは生体材料を抗菌性物質でコーティングするなどの工夫がなされてきた。一方でどれくらいの菌数があれば創部感染が引き起こされるのか明確にはなっておらず、今後の議論が必要である。
