背景

広範囲の微生物に対する短波長紫外線(UV-C)照射の有効性は、複数の研究で明らかにされているが、特異的な線量と微生物減少の程度の違いが確認された。さらに、実験室検査の状況は実際とは大きくかけはなれているため、検査で得られた有効性は、実際には必ずしも想定どおりになるとは限らない。したがって、代表的な実地試験において、UV-C の有効性を試験することが重要である。そこで、本研究では、用手消毒と比較して自動 UV-C を評価するために、実地試験を開発し構築することを目的とした。

方法

消毒済み表面と非消毒表面の代表的データセットを得るために、自然に高度に汚染された表面から、消毒前後にスワブ法によりサンプルを繰り返し採取した。その後、UV-C 照射とアルコールワイプによる完璧な用手による清拭消毒を実施し、対数減少値および消毒の達成を評価した。

結果

通常どおりほぼ均一に細菌によって自然に汚染された表面が、実地試験に特に適していると確認された。平均汚染は、UV-C 消毒では 23.3 cfu/cm2 から 1.98 cfu/cm2 (対数減少値 0.9)に減少、用手消毒では 29.7 cfu/cm2 から 0.26 cfu/cm2(対数減少値 1.2)に減少した。表面消毒の達成度が、UV-C 消毒では 75.5%であったのに対し、用手消毒では 98.1%であった。

結論

完璧な用手による清拭消毒は、自動 UV-C 消毒よりも、対数減少値および消毒の達成度がわずかに高いことが示された。技師に依存しない UV-C 消毒が達成できると、用手消毒に加えて、消毒効率をさらに改善する可能性がある。

監訳者コメント


COVID-19 の世界的流行以後 UV-C による環境消毒法が一般的になっているが、「医療現場での消毒交換に関する評価方法」が欧州や国際的な基準は定まっていない。UV-C については波長、線量、相対湿度や温度により効果が変化し、高度の汚染や影となる部分では効果がないなど、標準化のためには解決すべき課題が残されている。医療施設の環境は、さまざまな有機物や微生物で汚染されており、環境表面を UV-C のみで完全に消毒することは不可能であり、現時点では UV-C による環境消毒は低レベル消毒に分類されており、環境清拭消毒との組み合わせによる消毒が必要である。
