Long-term effect of carbapenem preauthorization: an interrupted time-series study over 20 years

背景

カルバペネムの事前承認は、カルバペネムの過剰使用の低減に有効と考えられているが、長期的評価は行われていない。

目的

日本の 3 次教育病院において、約 20 年間にわたる慎重なカルバペネム制限の長期的影響を評価することを目的とした。

方法

分割時系列(ITS)解析を用いて、介入前の期間(2004 年および 2005 年)と介入期間(2006 年から 2023 年)を比較し、カルバペネムの抗菌薬使用密度(AUD)の値および傾向の変化を検討した。さらに、入院患者のカルバペネムの AUD と、病院感染菌血症の死亡率および多剤耐性院内病原体による菌血症の有病率の関連性について探索した。

結果

ITS解析により、事前承認導入後のカルバペネムの AUD の値に顕著かつ有意な低下が示された(100 人日あたり−0.367、95%信頼区間[CI]−0.131 ~ −0.603、P = 0.002)。さらに、傾向の変化は 100 人日あたり −0.014 であった(95%CI、−0.001 ~ −0.028、P = 0.048)。カルバペネムの事前承認導入後、病院感染菌血症による 28 日死亡率の上昇は認められなかった。カルバペネムの AUD と、メチシリン耐性黄色ブドウ球菌(Staphylococcus aureus)(相関係数[ρ] = 0.77[P < 0.001])、メロペネム耐性緑膿菌(Pseudomonas aeruginosa)(ρ = 0.85[P < 0.001])、およびメロペネム耐性アシネトバクター・バウマニー(Acinetobacter baumannii)(ρ = 0.80[P < 0.001])の保菌率に、有意な正の相関が認められた。

結論

長期間のカルバペネム事前承認の実施は病院内の疫学に有効であり、有害な影響はないことが、20 年間の ITS解析によって一貫して証明された。

監訳者コメント

日本の大学病院でカルバペネム事前承認によりカルバペネムの使用が減少したが、菌血症の死亡率は上昇しなかった。カルバペネムの抗菌薬適正使用支援を推進する上で、非常に参考になる。
