Investigation on hand hygiene in anaesthesiology departments in China

目的

中国の病院の麻酔科スタッフにおける手指衛生の実践および知識の現状を調査して、医療従事者における手指衛生実践の標準を改善するための基準を提供すること。

方法

2024 年 11 月 1 日から 12 月 31 日に、中国の病院に勤務する麻酔科スタッフを対象に、オンライン全国横断調査を実施した。学際的チームが作成した質問票には、社会人口統計学、手指衛生実践、熟練度および知識が含まれた。回答 2,580 件のうち、質を検討した結果 2,512 件が適格とされた。記述統計、カイ二乗検定およびロジスティック回帰を用いてデータを解析し、手指衛生遵守の予測因子を同定した。

結果

本研究において、適格な回答 2,512 件を解析し、半数以上が男性（59.39％）、またほとんどが麻酔科医（90.84％）であった。過半数の回答者がクラスIIIA 病院（60.67％）の所属であった。ほとんどの回答者が正式の手指衛生トレーニングを受けている（94.43％）、また手指消毒剤を定期的に使用している（90.61％）と回答し、3 分の 2 以上が適切な消毒時間および洗浄時間（それぞれ 66.44％および 67.27％）および7 段階の手洗い手順への高い遵守（81.76％）を報告した。手指衛生遵守に対する阻害因子は、「忙しすぎる」（66.28％）、「ハンドタオルが十分に利用できない」（41.68％）、「手袋が手洗いの代わりになる」という考え（39.93％）、不十分な設備（18.91％）などであった。手指衛生の評価から、知識スコアの平均は 61.02、実践スコアの平均は 80.36、ならびに熟練度スコアは 82.62 であることが示された。知識および正式な訓練のレベルが高いほど、推奨された実践に対する遵守率が高かった。

結論

本研究から、中国の麻酔科スタッフにおいて手指衛生実践の改善が必要であることが浮き彫りになっており、訓練、知識、および手指衛生遵守に対する阻害因子への取り組みの必要性が強調されている。

監訳者コメント：

中国における麻酔科職員の手指衛生の状況をオンライン調査したものであり、自己申告である点で実際との乖離がある可能性は否めない。一方で、知識はあっても、実践にその知識が活かされていない点が明らかとなったが、麻酔科の医師がこの調査の大半であったことを考慮すると妥当な結果かもしれない。また、手指衛生の遵守率向上のためには、誤った知識の是正（業務多忙による手洗い時間の不足、手洗いを手袋での代用、患者ケアを手洗いより優先など）や職場環境（ペーパータオル、非接触の蛇口など）改善など、多面的なアプローチが必要である。
