Viral acute respiratory infections in neonatal intensive care healthcare workers: a nine-month point-prevalence cohort study

背景

医療関連ウイルス性急性呼吸器感染症は新生児のケアでよく見られる問題である。医療従事者は、症候性や無症候の急性呼吸器感染症に罹患しているとき、新生児にウイルスを伝播する可能性がある。

目的

この 9 か月間の前向き繰り返し点有病率コホート研究は、3 次の新生児集中治療室（NICU）の医療従事者における無症候および症候性の急性呼吸器感染症の発生および原因を調べることを目的とした。

方法

隔週の火曜日に、NICU で、その日に働いていた全職員から鼻腔のフロックスワブを採取した。それに加えて、急性呼吸器感染症の症状があった例では、被験者自身が鼻腔のスワブを採取した。

結果

無症候の被験者の 16 例（3.3％）でウイルスが検出された。合計で 36 件の症候性急性呼吸器感染症（平均で 0.5 件／人）が報告された。

結論

我々のデータは、NICU の医療従事者の間で、急性呼吸器感染症は稀ではなく、さらに、無症状のことも多いことを示唆している。これらの人々が伝播させる可能性があることに注目すべきである。

急性呼吸器感染症（ARI）のサーベイランスが 2025 年 4 月から日本でも開始され、情報が蓄積されつつある。本研究は NICUに 従事する医療従事者の無症状を含めたウイルス性ARI の 9 か月の積極的サーベイランス結果から、無症状感染者の存在による NICU 内での感染伝播が危惧されるところである。有症状時の数日間の患者ケアの回避義務、有症状時のマスク着用などの NICU での感染対策が効を奏したのか、期間中の入院患者への感染は観察されていない。ただし、NICU に限らず、すべての医療施設においては、「職員の感染症に関する就業ルール」を定めておくことで混乱なく業務が遂行でき、また休務者が増加してきた時点での業務制限など BCP についても言及しておくことも必要である。
