中和剤を添加して収集した十二指腸内視鏡培養の高い回収率★★

目的

微生物学的培養は、十二指腸内視鏡の再処理のモニタリングにおけるゴールドスタンダードである。しかし、多くの異なるサンプリングおよび培養方法が用いられており、結果の比較を困難にしている。米国疾病対策センターの最新のプロトコールでは、サンプル中に残存する消毒薬を不活性化するため、中和剤の使用を勧告している。本研究では、十二指腸内視鏡サンプルを、中和剤を添加して収集した場合と添加せずに収集した場合の培養結果を比較した。

方法

非臨床の実験環境において 6 台の十二指腸内視鏡を腸内細菌で汚染し、その後再処理を行った。汚染除去後または乾燥後に直ちに先端部および鉗子チャンネルのサンプルを収集した。4 台の十二指腸内視鏡のサンプルには中和剤として Dey-Engley ブロスを添加し、他の 2 台の十二指腸内視鏡のサンプルには Dey-Engley ブロスを添加せずに収集した。

結果

汚染除去後の培養物は、Dey-Engley ブロスを添加したサンプル群で適用した微生物の

増殖が陽性である傾向が有意に強かった(88.1%対 20.2%、P < 0.0001)。乾燥後のサンプルは、Dey-Engley ブロスを添加しなかったサンプル群で陽性である傾向が有意に強かった(75.7%対 33.4%、P < 0.001)。

結論

乾燥していない十二指腸内視鏡から収集したサンプルへ Dey-Engley ブロスを添加することによって、それらの培養物の回収率が高くなる。乾燥していない十二指腸内視鏡に残存する消毒薬は、偽陰性の結果を引き起こす可能性がある。これは、Dey-Engley ブロスのような中和剤をサンプルに添加することによって克服できる。中和剤を添加しなかった群の乾燥後の高い回収率は、これら 2 台の十二指腸内視鏡でのバイオフィルムの形成が原因である可能性があるが、このことは調査しなかった。サンプリング方法を標準化することによって、十二指腸内視鏡の汚染に関する臨床結果と研究結果の両方を比較するのに役立つ可能性がある。

監訳者コメント

内視鏡的逆行性胆管膵管造影(ERCP)等に用いられるもので、内視鏡の先端部の複雑な構造に起因して十分な洗浄・消毒等がなされず、多剤耐性菌(CRE など)が伝播した可能性が米国 FDA より 2015 年に安全情報として報告され、内視鏡の再生処理の手順が詳細に決められた。再生処理後のバリデーションとしての細菌培養は、CDC がその方法を提示しているが、実施については必須ではなく各州や各施設での取り決めに委ねられている。細菌培養の結果は残存消毒剤の中和の有無に左右されことが指摘され。本論文でも中和剤の添加を標準化することを提案している。H27 年 3 月 6 日、日本でも厚労省から通達がでている。https://www.jges.net/wp-content/uploads/2019/01/28fc4053fbd7fc8aef4c80967900afee.pdf
