背景

心臓手術後の手術部位感染症は、合併症や予定外の医療利用のよくある原因であり、ほとんどの感染症は退院後に発症する。スマートフォン技術と人工知能(AI)を用いた遠隔創傷モニタリングは、合併症のより早期の発見を支援する可能性がある。

目的

AI 対応デジタル創傷モニタリングプラットフォーム+通常診療の併用(Isla-AI)の実行可能性、受容性および安全性を、通常診療(UC)単独との比較により評価した。

デザイン、設定および参加者

この多施設共同 2 群ランダム化対照実行可能性試験は、2024 年 8 月から 2025 年 1 月に英国の 2 つの病院で実施した。心臓手術を受けた成人患者を、Isla-AI または UC を受ける群にランダムに割り付けた。本研究は、有効性を評価するための検出力は持たなかった。

結果

患者 120 例がランダム割付けされ、参加した(Isla-AI 群 N = 62、UC 群 N = 58)。実行可能性の目標は上回った。すなわち、参加を打診された適格患者の 60%が同意し、Isla-AI 群患者の 95%が少なくとも 1 枚の画像を提出し、92%が研究を完了した。画像の 98%が臨床評価に適していた。医師と AI が生成した優先フラグとの一致率は 87%であった。AI の優先順位付け性能は、肌の色がより浅黒い患者において若干優れていた。患者の半数以上が、創傷画像の取込みまたは提出に手助けを必要とした。患者およびスタッフの AI 受容性は大部分が良好であった。有害事象および重篤な有害事象の発現割合は、両群で同程度であった。創傷に関連した問題および抗菌薬のために、英国国民保健サービス(NHS)の医療資源を利用した患者の割合は、Isla-AI 群のほうが低かった。

結論

以上の結果から、公平性、使用性およびワークフローへの組込みにさらに配慮した、完全な大規模多施設共同有効性試験への進行が支持される。
