臨床環境および非臨床環境での多剤耐性微生物の空中伝播における手洗場の役割の評価★

背景

臨床現場における多剤耐性病原体(MDRO)の伝播はこれまで、汚染された表面や医療機器との接触に起因するとされてきたが、病院環境のその他の要素、特に手洗場も MDRO のリザーバーとして機能する可能性があることを示唆するエビデンスが増加している。しかし、臨床現場の条件下における手洗場からの MDRO の空気伝播については、十分に明らかにされていない。

方法

入院患者用病室 5 室および臨床で使用しない区域のトイレ 3 室において、蛇口操作時および対照条件下でエアロゾルおよび表面のサンプリングを実施した。エアロゾルの採取は、一段階のアンダーセンカスケードインパクターおよび BioSpot-VIVAS サンプラーを用いて行った。排水口のスワブサンプルも採取した。すべてのサンプルを選択培地および非選択培地で培養し、得られた分離株の同定を、マトリックス支援レーザー脱離イオン化飛行時間型質量分析法を用いて、カルバペネム耐性菌およびカンジダ(Candida)属菌に焦点を当てて行った。

結果

入院患者用病室およびトイレにおいて、蛇口操作時のエアロゾルサンプルで少なくとも 7 種類の MDRO が検出された。微生物の種類には、エアロゾルの2 つのサンプリング法の間で大きな差があった。複数の部屋で、エアロゾルと排水口のスワブにおいて共通の微生物が検出された。

結論

手洗場での作業は、生きた MDRO のエアロゾル発生に寄与する可能性があり、このことから、感染予防戦略において考慮する必要のある、可能性のある伝播経路が浮き彫りになっている。さらに、空中病原体の包括的な評価を行うために複数のエアロゾル採取方法を検討する必要がある。

監訳者コメント:
実際の病室で、蛇口作動中の空気とシンク排水口から同じ菌種の MDRO が検出され、シンクが飛沫・エアロゾル発生源となる可能性が示された。ただし、最大流量を 30 ~ 60 分継続する条件は通常使用と異なり、粒径・菌量の定量や WGS による同一株確認も行われていない。本研究が証明したのは生菌の空気中放出までであり、患者への「空気感染」や排水口由来の伝播を直接示したものではないことに注意が必要である。
