手術室の手作業での環境消毒における過酸化水素および銀イオンと次亜塩素酸ナトリウムの比較:残存抗菌効果についての多施設共同無作為化試験★

背景

手術室の環境表面の汚染は、手術部位感染症と関連する病原体の伝播に関与している。次亜塩素酸ナトリウムは、環境消毒で広く使われているが、抗菌活性が残らないため、表面が急速に再汚染される可能性がある。この研究では、手術室の高頻度接触表面に対する過酸化水素・銀イオン製剤の即時の抗菌効果および残存する抗菌効果を次亜塩素酸ナトリウムと比較した。

方法

この多施設共同無作為化実験研究では、3 つの三次病院の 24 の手術室を、銀イオン(10 ppm)で安定化させた過酸化水素(2%)による消毒と、次亜塩素酸ナトリウム(0.5%)による消毒のどちらかに割り付けた。5 つの高頻度接触表面から、ベースライン、消毒直後、消毒から 2 および 4 時間後に、合計で 480 個の検体を採取した。微生物量および再汚染率を、統計学的に比較した。

結果

ベースラインの微生物量は、両群で同程度であった(P = 0.115)。そして、どちらの消毒剤も、即時の完全な細菌の減少を達成した。消毒から 4 時間後の時点で、次亜塩素酸ナトリウムで処理した表面には、かなりの再汚染がみられた一方で(60.0%、中央値、20.0 cfu/mL)、過酸化水素・銀イオンで処理した表面では、微生物の抑制の維持が示された(8.3%、中央値、0.00 cfu/mL、P < 0.001)。緑膿菌(Pseudomonas aeruginosa)の再増殖が、次亜塩素酸ナトリウム群のみで観察された。

結論

即時の抗菌効果は同程度であったにもかかわらず、次亜塩素酸ナトリウムには残存活性がなく、表面が急速に再汚染されることを許した。過酸化水素・銀イオン製剤は、4 時間にわたって、優れた残存抗菌作用を提供したことから、外科処置が長引いた際の感染予防に使用できる可能性が示される。

監訳者コメント

消毒直後だけでなく 4 時間後の再汚染を評価しており、過酸化水素・銀イオン製剤は手術室の環境衛生の維持につながる可能性がある。手術室の持続的環境消毒の導入を検討する際の基礎データである。
