ESKAPE 病原体に関連した院内感染の負担:システマティックレビューによる臨床的および経済的観点★

抗菌薬耐性(AMR)は、ESKAPE 病原体に起因する院内感染(HAI)の管理に重大な課題をもたらす。本研究では、Embase、Web of Science および PubMed において、2000 年から 2023 年に発表された論文を対象に系統的文献検索を行い、ESKAPE 感染症の臨床的および経済的負担を評価した。エビデンスには死亡率、入院期間(LOS)および医療費が含まれ、ほとんどの研究が、メチシリン耐性黄色ブドウ球菌(Staphylococcus aureus)(MRSA)に起因する HAI に焦点を合わせていた。これらの影響は、地域、ESKAPE 病原菌株および耐性の種類によって異なった。非耐性群と比較して、耐性 ESKAPE 関連 HAI は、ほぼ 2倍の死亡リスクに関連した(オッズ比 = 1.96、95%信頼区間 = 1.56 ~ 2.46)。耐性 ESKAPE 関連 HAI はさらに、特にアジア太平洋地域において、病院 LOS を有意に長期化させ(P < 0.001)、医療費を増加させた(P = 0.047)。本研究では、特に資源が限られた環境において、AMR の影響を軽減するためには、包括的戦略と世界的協力が差し迫って必要であることが浮き彫りになっている。これらの課題に取り組むことで、患者転帰が改善され、世界中の医療システムにかかる経済的負担が軽減される可能性がある。

これらは、医療関連感染症の原因となる感染症の重要な耐性菌となりうる。
