背景

新興超多剤耐性菌(eXDR)、とりわけカルバペネマーゼ産生菌(CPO)やバンコマイシン耐性腸球菌の保菌者を迅速に検出することは、それらの拡散を抑え、感染を防ぐために極めて重要である。

目的

eXDR の検出の所要時間(TAT)に対するポリメラーゼ連鎖反応(PCR)の影響を、夜間や週末での分析を除いた通常の臨床検査の条件下での培養による方法と比較して評価すること。

方法

前向き前後研究をフランスの大学病院で実施した。第一相(培養)は、2022 年 2 月から 9 月まで、第二相(PCR)は 2022 年 9月から 2023 年 3 月までであった。TAT は、検体の採取から結果が感染制御チームおよび/または医師に届けられるまでの時間と定義した。年齢が 18 歳を超えていて、特異的なスクリーニング基準を満たしている患者を組み入れた。BD MAX Check-Points CPO キットと VIASURE Vancomycin resistance Real Time PCR Detection キットという 2 つの PCR キットを使用した。

結果

合計で、第一相に 2,049 例、第二相に 1,861 例の患者を組み入れた。検査で初めて eXDR が陽性となった患者は、第一相で 16 例、第二相で 25 例であった。TAT の中央値は、第二相が(22.7 時間、16.0 ~ 41.3)、第一相(43.5 時間、31.9 ~ 69.7、P = 0.001)と比べて、有意に短かった。第二相では、第一相と比べて、接触患者数の中央値の有意な減少が観察された。スクリーニング検査での二次症例の総数や入院費は、eXDR の検出方法に関係がなく同程度であった。

結論

あまり流行していない状況では、PCR を使った迅速直腸スクリーニングによって、費用が増加することなく、結果までの TAT が有意に減少する。

監訳者コメント :

PCR 検査を用いた薬剤耐性菌の迅速な直腸スクリーニングにより、低流行地域における新規陽性患者において、TAT の短縮が期待される。
